HTN Adult Initial or maintenance dose 8 mg once daily. May be increased to 16 mg once daily & up to max 32 mg once daily.
Elderly No initial dose adjustment.
Patients w/ intravascular vol depletion Initially 4 mg.
Patient w/ renal impairment Initially 4 mg.
Patient w/ mild to moderate hepatic impairment Initially 4 mg once daily.
Heart failure Adult Initially 4 mg once daily. May be increased up to max 32 mg once daily by doubling the dose at 2 wk intervals.
Elderly No initial dose adjustment.
