Indications/Uses
Treatment of patients w/ DM who require insulin for the maintenance of glucose homeostasis.
Dosage/Direction for Use
Contraindications
Hypersensitivity. Hypoglycaemia. Under no circumstances should any Humulin formulation other than Humulin R be given IV.
Special Precautions
Transferring from another type or brand of insulin or switching from animal insulin to human insulin. Conditions which may make the early warning symptoms of hypoglycaemia less pronounced, including long-duration diabetes, diabetic nerve disease or medications eg, β-blockers. Formation of Ab. Patient w/ diseases of adrenal, pituitary, or thyroid glands; renal or hepatic impairment; illness or emotional disturbances; changes in level of physical activity or usual diet. Perform continuous rotation of inj site to reduce risk of developing lipodystrophy & cutaneous amyloidosis. Observe for signs & symptoms of heart failure, wt gain & oedema if therapy is in combination w/ pioglitazone, especially in patients w/ risk factors for development of cardiac heart failure. May impair ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Hypoglycaemia, local allergy (redness, swelling & itching at the site of insulin inj); oedema. Systemic allergy, lipodystrophy.
Drug Interactions
Insulin requirements may be increased by substances w/ hyperglycaemic activity eg, glucocorticoids, thyroid hormones, growth hormone, danazol, β2-sympathomimetics (eg, ritodrine, salbutamol, terbutaline), thiazides. Insulin requirements may be reduced by drugs w/ hypoglycaemic activity eg, oral hypoglycaemics, salicylates (eg, ASA), certain antidepressants (MAOIs), certain ACE inhibitors (captopril, enalapril), angiotensin II receptor blockers, non-selective β-blocking agents & alcohol. Somatostatin analogues (octreotide, lanreotide) may both decrease or increase insulin dose requirements.
