Indications/Uses
Treatment of infections in adults & childn ≥3 mth: Acute streptococcal tonsillitis & pharyngitis; acute bacterial sinusitis; acute otitis media; acute exacerbations of chronic bronchitis; cystitis; pyelonephritis; uncomplicated skin & soft tissue infections; early Lyme disease.
Dosage/Direction for Use
Usual course of therapy is 7 days (range: 5-10 days). Adult & childn ≥40 kg Acute tonsillitis & pharyngitis, acute bacterial sinusitis; cystitis, pyelonephritis; uncomplicated skin & soft tissue infections 250 mg bd. Acute otitis media; acute exacerbations of chronic bronchitis 500 mg bd. Lyme disease 500 mg bd for 14 days (range of 10-21 days). Childn <40 kg Acute tonsillitis & pharyngitis, acute bacterial sinusitis 10 mg/kg bd to a max of 125 mg bd. Otitis media or more severe infection in childn ≥2 yr; cystitis; uncomplicated skin & soft tissue infections 15 mg/kg bd to a max of 250 mg bd. Pyelonephritis 15 mg/kg bd to a max of 250 mg bd for 10-14 days. Lyme disease 15 mg/kg bd to a max of 250 mg bd for 14 days (10-21 days). Renal impairment ≥30 mL/min/1.73 m2 Standard dose of 125-500 mg given bd, 10-29 mL/min/1.73 m2 Standard individual dose given every 24 hr, <10 mL/min/1.73 m2 Standard individual dose given every 48 hr. During haemodialysis A single additional standard individual dose should be given at the end of each dialysis.
Administration
Should be taken with food: Take after food for optimum absorption. Tab: Do not crush/chew.
Contraindications
Hypersensitivity to cefuroxime, or to cephalosporins. History of severe hypersensitivity to any other type of β-lactam antibacterial agents (penicillins, monobactams, carbapenems).
Special Precautions
Patients w/ history of non-severe hypersensitivity to other β-lactam agents. Immediately discontinue in case of severe hypersensitivity reactions. Jarisch-Herxheimer reaction following cefuroxime axteil treatment of Lyme disease. Overgrowth of Candida. Prolonged use may result in overgrowth of other non-susceptible organisms (eg, Enterococci, Clostridium difficile). Antibacterial agent-associated pseudomembranous colitis. +ve Coombs' test. False -ve result in the ferricyanide test. May affect ability to drive or operate machinery. Patient w/ renal impairment. Pregnancy & lactation. Tab: Contains parabens. Susp: Contains sucrose & aspartame.
Adverse Reactions
Overgrowth of Candida; eosinophilia; headache, dizziness; diarrhoea, nausea, abdominal pain; transient increase in hepatic enzyme levels.
Drug Interactions
Lower bioavailability w/ drugs which reduce gastric acidity. May affect gut flora, leading to lower oestrogen reabsorption & reduced combined OC efficacy. Increased peak conc, AUC & elimination t½ w/ probenecid. Increased INR w/ oral anticoagulants.
