Phytomenadione.
Action
Phytomenadione (vitamin K) is needed for the formation of blood clotting factors in the liver such as factor II (prothrombin), VII, IX and X. Phytomenadione (vitamin K) acts as a coenzyme in carboxylation of side chains containing glutamic acid. The resulting Ɣ-carboxy glutamyl compound converts the precursor into an active clotting factor which is then released by the liver cells into the blood.
Indications/Uses
Prophylaxis and treatment of hemorrhage in newborn.
Dosage/Direction for Use
Hemorrhage prophylaxis in newborns: 0.5 mg-1 mg Phytomenadione (vitamin K) is administrated by IM, 1-6 hours after the baby was born.
Treatment of hemorrhage in newborns: 1 mg Phytomenadione (vitamin K), given by IM or subcutaneously.
Contraindications
Hypersensitivity.
Special Precautions
The coagulation effect of Phytomenadione (vitamin K) will be produced 1-2 hours after the drug is given.
Should not exceed the recommended dosage.
Phytomenadione (vitamin K) is an antagonist of anticoagulant, coumarin but cannot inhibit the action of anticoagulants from heparin
Phytomenadione (vitamin K) is easily degraded by light, keep the preparation in a place protected from light.
Intravenous administration should be avoided.
Adverse Reactions
Hyperbilirubinemia is reported to have occurred in a newborn if the drug is given excessively from the recommended dose.
Cyanosis, peripheral vascular collapse, hot flushes, sweating, chest pain, hyperhidrosis, shock, hypersensitivity reactions, including anaphylactic reactions and deaths reported by intravenous administration. Giving intravenously should be avoided.
Local irritations such as pain, swelling and tenderness may occur to the recent injection site.
Parenteral administration of newborns / neonates may cause hemolytic anemia and hemoglobinuria.
