Indications/Uses
Prevention of artherothrombotic events eg, recent MI, stroke, established peripheral arterial disease; acute coronary syndrome; patients suffering from MI (from few days until <35 days), ischaemic strokes (from 7 days until <6 mth); non-ST segment elevation acute coronary syndrome (unstable angina or non-Q-wave MI) including patients undergoing stent replacement following percutaneous coronary intervention. In combination w/ ASA for ST segment elevation acute MI in medically treated-patients & eligible for thrombolytic therapy.
Dosage/Direction for Use
Adult & elderly 75 mg as single daily dose. Non-ST segment elevation acute coronary syndrome (unstable angina or non-Q-wave MI) Initially 300 mg as single loading dose then continued at 75 mg once daily w/ ASA 75-325 mg daily. ST segment elevation acute MI 75 mg as single daily dose initiated w/ loading dose in combination w/ ASA & w/ or w/o thrombolytics.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Active pathological bleeding eg, peptic ulcer or intracranial hemorrhage. Severe hepatic impairment. Lactation.
Special Precautions
Genetically reduced CYP2C19 function (intermediate or poor metabolisers); unusual bleeding; if bleeding occurs during the course of treatment; patients from trauma, surgery or other pathological conditions; lesions w/ propensity to bleed particularly GI & intraocular. Discontinue use 7 days prior to elective surgery. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Renal impairment. Moderate hepatic disease. Pregnancy & lactation.
Adverse Reactions
Eye bleeding (conjunctival, ocular, retinal), haematoma, epistaxis, GI disturbances, bleeding at puncture site, bruising.
Drug Interactions
Reduded clinical efficacy w/ CYP2C19 eg, PPIs. Increased bleeding intensity w/ warfarin & glycoprotein IIb/IIIa inhibitors, heparin & thrombolytics. Increased occult GI blood loss w/ other NSAIDs including COX-2 inhibitors.
