Indications/Uses
Treatment of bone & joint infection, bronchitis, gonorrhea, meningitis, otitis media, peritonitis, pharyngitis, sinusitis, skin disorders. Prophylaxis for surgical infection & UTI.
Dosage/Direction for Use
Adult 750 mg 8 hrly. More severe infections 1.5 g IV 6 or 8 hrly. CrCl 10-20 mL/min 750 mg bid. CrCl <10 mL/min 750 mg once daily. Infant & childn 25-100 mg/kg/day in 3 or 4 divided doses. A dose of 60 mg/kg/day is appropriate for most infections. Neonate 30-100 mg/kg/day in 2 or 3 divided doses in the 1st wk of life. Gonorrhea 1.5 g as a single dose or two 750-mg inj in 2 different sites. Meningitis Adult 3 g IV 8 hrly. Childn & infant 200-240 mg/kg IV daily in 3-4 divided doses, may be decreased to 100 mg/kg daily after 3 days or when clinical improvement is evident. Neonate Initially 100 mg/kg/day IV, may be reduced to 50 mg/kg/day IV. Prophylaxis for abdominal, pelvic & orthopedic operation 1.5 g IV w/ induction of anesth, may be supplemented w/ 2 x 750 mg IM after 8 & 16 hr. Cardiac, pulmonary, esophageal & vascular operation 1.5 g IV w/ induction of anesth continuing w/ 750 mg IM tid for further 24-48 hr. Total joint replacement 1.5 g mixed dry w/ each pack of methyl methacrylate cement polymer before adding the liqd monomer. Impaired renal function Adult 750 mg bid. Severe impairment 750 mg once daily. Patient on dialysis 750 mg given at the end of each dialysis. Continuous peritoneal dialysis 750 mg bid.
Contraindications
Hypersensitivity to other cephalosporins.
Special Precautions
Renal impairment. Monitor renal & hematological status in prolonged & high-dose therapy.
Adverse Reactions
Skin rashes, urticaria, eosinophilia, fever, serum sickness-like reactions, anaphylaxis. Bleeding complication related to hypothrombinemia &/or platelet dysfunction. Nephrotoxicity. Acute renal tubular necrosis. Prolonged use. Pseudomembranous colitis.
