Glibenclamide
Dosage/Direction for Use
Initially 2.5-5 mg daily. May adjust dose at intervals w/ 2.5 mg increments. Max: 20 mg daily. Doses >10 mg daily may be given in divided doses.
Administration
Should be taken with food: Take immediately before breakfast.
Contraindications
Insulin dependent DM or Type I DM. NIDDM. Porphyria; ketoacidosis, stress, severe infection, trauma or other severe conditions. Hepatic & renal insufficiency/failure. Pregnancy & lactation.
Special Precautions
Patients w/ CV disease; debilitated, malnourished & adrenal or pituitary insufficiency. May impair ability to drive or operate machinery. Renal & hepatic impairment. Fasting patients. Elderly.
Adverse Reactions
Headache, sleepiness, dizziness, nausea, vomiting, heartburn, diarrhea, constipation, abdominal pain, pruritus, rashes & photosensitivity; cholestatic jaundice, eosinophilia, transient leukopenia, thrombocytopenia, aplastic anemia, agranulocytosis, haemolytic anemia, erythema multiforme, & exfoliative dermatitis; hypoglycemic coma; increased appetite, wt gain, enhance water clearance.
Drug Interactions
Reduced effect w/ epinephrine, chlorpromazine, corticosteroids, OCs, thiazides & thyroid hormone. Reduced hypoglycemic effect w/ rifampicin. Hypoglycemic effect may be heightened by ACE inhibitors, alcohol, allopurinol, selected analgesics eg, phenylbutazone & salicylates, ketoconazole, fluconazole & miconazole, chloramphenicol, cimetidine, clofibrate, coumarins, fluoroquinolones, heparin, MAOIs, ranitidine, sulphonamides, tetracyclines & TCAs. May increase effect & mask typical sympathetic warning signs w/ β-blockers.
