Amlodipine
Indications/Uses
1st line treatment of HTN as monotherapy or in combination w diuretics, α-blockers, β-blockers or an ACE inhibitors. Symptomatic treatment of chronic stable angina or confirmed or suspected vasospastic (Prinzmetal's) angina as monotherapy or in combination w/ antianginal drugs.
Dosage/Direction for Use
Adult Initially 5 mg once daily, may be increased to a max dose of 10 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to amlodipine or to dihydropyridine derivatives. Severe hypotension; shock (including cardiogenic shock); obstruction of the outflow tract of left ventricle (eg, high grade aortic stenosis); haemodynamically unstable heart failure after acute MI.
Special Precautions
Severe heart failure (NYHA grade III & IV), CHF; hepatic impairment. May affect ability to drive or operate machinery. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Somnolence, dizziness, headache; palpitations; flushing; abdominal pain, nausea; ankle swelling; oedema, fatigue.
Drug Interactions
Increased exposure of simvastatin. May increased bioavailability w/ grapefruit juice. Increased systemic exposure w/ diltiazem. Increased plasma conc w/ strong CYP3A4 inhibitors (eg, ketoconazole, itraconazole, ritonavir). Reduced plasma conc w/ CYP3A4 inducers (eg, rifampicin, St. John's wort).
