Isotretinoin
Indications/Uses
Severe forms of nodulocystic acne resistant to previous therapy, particularly cystic acne & acne conglobata, especially when the lesions involve the trunk.
Dosage/Direction for Use
Adult & adolescent 0.5-1 mg/kg body wt/day in 2 divided doses for 15-20 wk. Max: 2 mg/kg body wt/day. Treatment duration: 16 wk. Treatment may be resumed after an interval of at least 8 wk.
Administration
Should be taken with food.
Contraindications
Hepatic & renal insufficiency; hypervitaminosis A; excessively elevated blood lipid values. Blood donation during or w/in 1 mth of cessation of isotretinoin to women of childbearing potential. Pregnancy & lactation.
Special Precautions
Regularly check liver function & serum lipids (fasting value). History of depression; monitor signs of depression. Avoid contraceptives containing androgen progestational substance; dermabrasion, wax epilation; supplementary treatment w/ tetracyclines. Check relevant lab parameters especially in patients w/ diabetes, obesity, alcoholism or lipid metabolism disorders.
Adverse Reactions
Cheilitis, epistaxis, skin infection, dryness of skin & mucosa, muscle & joint pain, depression, headache, increased intracranial pressure, seizures, visual disturbances, nausea, inflammatory bowel disease, increase in transaminases, hepatitis, bronchospasm, blood disorders.
Drug Interactions
Vit A, progesterone prep, tetracyclines.
