Amlodipine (equiv to amlodipine besylate)
Indications/Uses
Monotherapy or in combination w/ other antihypertensive agents in HTN. Reduce risk of coronary revascularization & the need for hospitalization due to angina in patients w/ CAD. 1st-line treatment of myocardial ischemia whether due to fixed obstruction (stable angina) &/or vasospasm/vasoconstriction (Prinzmetal's or variant angina) of coronary vasculature as monotherapy or in combination w/ other antianginal drugs.
Dosage/Direction for Use
HTN Adult Usual initial dose: 5 mg once daily. Max dose: 10 mg once daily. Childn 6-17 yr 2.5-5 mg once daily. Elderly 2.5 mg once daily. Hepatic function impairment Initially 2.5 mg once daily. Titrate slowly in patients w/ severe hepatic impairment. CAD Adult 5-10 mg once daily. Chronic stable angina Adult 5-10 mg once daily. Most patients will require 10 mg for adequate effect. Elderly & hepatic function impairment 5 mg once daily.
Administration
May be taken with or without food.
Special Precautions
Hepatic impairment. Symptomatic hypotension may occur especially in patients w/ severe aortic stenosis. Worsening of angina or acute MI may occur particularly in patients w/ severe obstructive CAD upon initiation of therapy or dose increase. Pregnancy & lactation.
Adverse Reactions
Edema, dizziness, flushing, palpitations, fatigue, nausea, abdominal pain, & somnolence.
Drug Interactions
Increased systemic exposure w/ moderate (eg, diltiazem) or potent (eg, clarithromycin, itraconazole) CYP 3A inhibitors. Increase exposure of simvastatin. Lower plasma conc w/ CYP3A4 inducers (eg, rifampicin, Hypericum perforatum).
