Indications/Uses
Symptomatic treatment of mild, moderate & severe Alzheimer's disease; vascular dementia (dementia-associated w/ cerebrovascular disease); dementia w/ Lewy bodies (DLB).
Dosage/Direction for Use
Adult & elderly Initially 5 mg/day in the evening (prior to retiring). May be increased to 10 mg/day after at least 4-6 wk of clinical assessment in patients who tolerated treatment at 5 mg/day. Vascular dementia & DLB Max: 10 mg/day.
Administration
May be taken with or without food: Take at bedtime. Place tab on the tongue & allow to disintegrate before swallowing w/ or w/o water.
Contraindications
Hypersensitivity to donepezil HCl or piperidine derivatives. Pregnancy.
Special Precautions
May exaggerate succinylcholine-type muscle relaxation during anesth. Patients w/ sick-sinus syndrome, supraventricular cardiac conduction conditions eg, SA or AV block, history of ulcer disease or those receiving concurrent NSAIDs; history of asthma or obstructive pulmonary disease. May exacerbate or induce extrapyramidal symptoms; cause bladder outflow obstruction. Avoid concomitant use w/ inhibitors of acetylcholinesterase, agonists or antagonists of the cholinergic system. Severe hepatic impairment. May affect ability to drive or operate machinery. Pregnancy & lactation. Not recommended for childn.
Adverse Reactions
Diarrhoea, nausea; headache. Common cold; anorexia; hallucinations, agitation, aggressive behaviour; syncope, dizziness, insomnia; vomiting, abdominal disturbance; rash, pruritis; muscle cramps; urinary incontinence; fatigue, pain; accident.
Drug Interactions
CYP3A4 (itraconazole & erythromycin) & CYP2D6 inhibitors (fluoxetine) may inhibit donepezil metabolism. Reduced levels w/ rifampicin, phenytoin, carbamazepine & alcohol. May interfere w/ drugs having anticholinergic activity. Concomitant use w/ succinylcholine, other neuromuscular & β-blocking agents, & cholinergic agonists may produce synergistic activity on cardiac conduction.
