Colistin (equiv to colistimethate Na)
Indications/Uses
Infections due to sensitive strains of certain gm -ve bacilli which are resistant to other antibacterial (eg, Acinetobacter spp., Citrobacter spp., E. coli, Enterobacter spp., H. influenzae, Klebsiella pneumoniae, Pseudomonas aeruginosa, Salmonella spp., Shigella spp., & some strains of Bordetella spp. & Vibrio spp).
Dosage/Direction for Use
2.5-5 mg/kg/day IV or IV infusion or IM in 2-4 divided doses depending upon the severity of the infection. Max: 5 mg/kg/day. Normal renal function (0.7-1.2 mg/dL creatinine & 80-100% urea clearance) 100-150 bid-qid. Approx daily dose: 5 mg/kg. Total daily dose: 300 mg. Mild renal impairment (1.3-1.5 mg/dL creatinine & 40-70% urea clearance) 75-115 mg bid. Approx daily dose: 2.5-3.8 mg/kg. Total daily dose: 150-230 mg. Moderate renal impairment (1.6-2.5 mg/dL creatinine & 25-40% urea clearance) 66-150 mg once daily or bid. Approx daily dose: 2.5 mg/kg. Total daily dose: 133-150 mg. Severe renal impairment (2.6-4 mg/dL creatinine & 10-25% urea clearance) 100-150 mg every 36 hr. Approx daily dose: 1.5 mg/kg. Total daily dose: 100 mg.
Special Precautions
Preexisting renal disease or elderly. May increase risk for neuromuscular blockade & apnea in impaired renal function. Transient, reversible neurological disturbances (eg, dizziness, numbness, paresthesia, tingling, & vertigo). Prolonged use may result in fungal or bacterial superinfection including C. difficile-associated diarrhea & pseudomembranous colitis. May impair ability to drive or operate machinery. Pregnancy & lactation.
Adverse Reactions
Dizziness, fever, headache, slurred speech, vertigo; pruritus, rash, urticaria; GI upset; paresthesia (extremities, oral), weakness (lower limb); increased BUN, increased creatinine, nephrotoxicity, proteinuria, decreased urine output; apnea, resp arrest.
Drug Interactions
Additive nephrotoxic &/or neurotoxic effects w/ aminoglycosides, amphotericin B, capreomycin, methoxyflurane, polymycin B sulfate, vancomycin). Potentiation of neuromuscular blockade w/ neuromuscular blocking agents eg, tubocurarine, succinylcholine, ether, decamethonium, gallamine & Na citrate.
