Indications/Uses
Arterial HTN; CHF. In combination w/ indapamide to reduce risk of stroke recurrence in patients w/ history of stroke or transient ischemic attack (TIA). Risk reduction of CV events in patients w/ stable CAD, on top of other preventive medications.
Dosage/Direction for Use
Adult Arterial HTN 5 mg daily in the morning. May be increased to 10 mg daily in single dose after 1 mth of treatment. CHF Initially 2.5 mg daily in the morning. May be increased up to 5 mg daily once BP acceptability has been demonstrated. Prevention of stroke recurrence in patients w/ history of cerebrovascular disease 2.5 mg daily for 2 wk, then increase to 5 mg daily for another 2 wk before introducing indapamide. Treatment initiation: Any time from 2 wk up to several yr after initial stroke episode. Risk reduction of CV event in patients w/ stable CAD 5 mg once daily for 2 wk, then increased up to 10 mg once daily. Elderly Arterial HTN Initially 2.5 mg daily in the morning. May be increased to 5 mg daily after 1 mth of treatment. Risk reduction of CV event 2.5 mg once daily for 1 wk, then 5 mg once daily the next wk before increasing to 10 mg once daily.
Administration
Should be taken on an empty stomach.
Contraindications
Hypersensitivity including angioneurotic oedema-type reactions. Diabetes. Extracorporeal treatments leading to contact of blood w/ negatively charged surfaces. Significant bilateral renal artery stenosis or stenosis of artery to single functioning kidney. Patients treated w/ aliskiren. Concomitant use w/ sacubitril/valsartan; K-sparing diuretics, K salts, lithium & estramustine. Impaired kidney function. Pregnancy (2nd & 3rd trimester) & lactation.
Special Precautions
Discontinue treatment if symptoms of angioedema develop. Not recommended in primary aldosteronism. Not to initiate sacubitril/valsartan until 36 hr after last perindopril dose. Increased risk of hypotension & renal insufficiency. Aortic stenosis, hypertrophic cardiomyopathy or renal artery stenosis; other heart problems; collagen vascular disease eg, SLE or scleroderma; diabetes; recent diarrhoea or vomiting or dehydration; sugar intolerance; phenylketonuria; hyperkalaemia. Patients to undergo anaesth &/or major surgery; LDL apheresis; treatment to reduce sensitivity to allergy to bee or wasp stings. Salt-restricted diet or use of salt substitutes containing K. Concomitant use of ARBs, aliskiren; drugs that increase serum K eg, heparin, co-trimoxazole; racecadotril; sirolimus, everolimus, temsirolimus & other mTor inhibitors. May affect driving or operating machinery. Liver or kidney problems. Patients receiving dialysis. Not recommended in early pregnancy & not to be taken if >3 mth pregnant. Not recommended during lactation. Black people. Not recommended in childn & adolescents ≤18 yr.
Adverse Reactions
Severe dizziness or fainting & light-headedness due to low BP, headache, dizziness, vertigo, pins & needles, vision disturbances, tinnitus, cough, shortness of breath, GI disorders, allergic reactions, muscle cramps, feeling of weakness. Erythema multiforme. Raynaud's phenomenon.
Drug Interactions
Concomitant use w/ ARBs, aliskiren, diuretics; K-sparing diuretics (eg, triamterene, amiloride, eplerenone, spironolactone), K supplements or K-containing salt substitutes; lithium; NSAIDs; insulin or metformin; baclofen; TCAs, antipsychotics; immunosuppressants eg, ciclosporin, tacrolimus; trimethoprim; estramustine; allopurinol; procainamide; vasodilators including nitrates; heparin; ephedrine, noradrenaline or adrenaline; IV gold salts; extracorporeal treatments. Increased risk of angioedema w/ racecadotril, mTOR inhibitors eg, sirolimus, everolimus, temsirolimus; sacubitril/valsartan. Increased hyperkalaemia w/ co-trimoxazole.
