Indications/Uses
Oedema due to cardiac, hepatic (ascites) or renal disease; burns. Acute cardiac insufficiency especially in pulmonary oedema. Reduce urinary output due to gestosis. In addition to other antihypertensives for hypertensive crisis. Supportive measures in brain oedema. To support forced diuresis in poisoning.
Dosage/Direction for Use
IM/IV Adult & adolescent ≥15 yr Initially 20-40 mg. If diuretic effect is not satisfactory, may be stepwise increased at 2-hrly interval by 20 mg each time until satisfactory diuresis is obtained. Give individual dose once daily to bid. Acute pulmonary oedema Initially 40 mg IV. Inj further dose of 20-40 mg after 20 min depending on patient's condition. Forced diuresis Give 20-40 mg in addition to infusion of electrolyte soln. Childn <15 yr & infant Life-threatening conditions 1 mg/kg up to max of 20 mg daily.
Contraindications
Hypersensitivity to furosemide or sulfonamides. Anuria, hepatic coma, hypokalaemia, hyponatraemia, hypovolaemia w/ or w/o hypotension.
Special Precautions
Concomitant use w/ alcohol. May impair ability to drive or operate machinery. Pregnancy. Discontinue lactation during treatment.
Adverse Reactions
Disturbed electrolyte & water balance; circulatory disturbances especially in elderly eg, feeling of pressure in the head, dizziness, mouth dryness, visual impairment; dehydration, circulatory collapse, thrombophilia; K-depletion; reduced serum Ca; GI disorders (eg, nausea, vomiting, diarrhoea), allergic reactions (eg, rashes, vasculitis, fever, interstitial nephritis); leukopenia, agranulocytosis, anaemia, thrombocytopenia; hydronephrosis, prostatic hypertrophy, ureterostenosis; transient rise in serum creatinine & urea; increased uric acid conc; rise in serum cholesterol & triglyceride levels; acute pancreatitis; aggravated metabolic alkalosis.
Drug Interactions
Concomitant use w/ cardiac glycoside. Increased K loss w/ glucocorticoids or laxatives. May potentiate nephrotoxic & ototoxic effects of aminoglycosides eg, kanamycin, gentamicin, tobramycin. Risk of inducing hearing defects w/ cisplatin. Diminished potency of antidiabetics & pressor amines. Potentiated effects of salicylates, theophylline, lithium & curarimimetic muscle relaxants; other hypotensive drugs especially ACE inhibitors. Antagonised action w/ NSAIDs eg, indomethacin.
