Pharmacology: Pharmacodynamics: Reduced viscosity of mucopurulent secretions: Mucotic HD contains Acetylcysteine is the N-acetyl derivative of the naturally occurring amino acid L-cysteine. Acetylcysteine reduces the viscosity of purulent and nonpurulent pulmonary secretions, through its free sulfhydryl group by cleaving the intramolecular disulfide bonds in mucoproteins aggregates.
Reduced toxicity of overdoses Paracetamol: Acetylcysteine is considered to reduce the hepatic toxicity, toxic intermediate metabolite following ingestion of a high dose of paracetamol. Acetylcysteine being provided with sulfhydryl group for maintains cellular glutathione at a level sufficient to conjugate with the toxic intermediate metabolite.

MUCOTIC-HD is used as an adjunctive mucolytic therapy, reduce the viscosity of the secretion from respiratory tract in respiratory disorders associated with productive cough, acute and chronic bronchitis and other respiratory conditions associated with the production of viscous mucus.
Antidotes for acute poisoning: MUCOTIC-HD is used as an antidote for the treatment of paracetamol overdosage.
MUCOTIC HD is administered orally.
Dosage: Reduced viscosity of the secretion: Adults and children over 7 years of aged: Dissolve 1 sachet in about half a glass of water (125 ml) once daily or in 3 divided doses.
Antidotes of Paracetamol: Acetylcysteine is used orally or IV as an antidote for the treatment of Paracetamol overdosage. When administered within 8 hours of Paracetamol ingestion, oral or IV acetylcysteine can effectively prevent or minimize hepatotoxicity associated with acute overdosage of Paracetamol. Acetylcysteine is most effective if administered within 8 hours of ingestion. However, acetylcysteine also may be effective when given 24 hours or longer after ingestion. When acetylcysteine is administered orally as a 5% solution, a loading dose of 140 mg/kg is administered. To complete a full course of therapy, the loading dose is followed by oral maintenance doses of 70 mg/kg every 4 hours for 17 doses. If a patient receiving oral acetylcysteine vomits a loading or maintenance dose within 1 hour of administration, the dose should be repeated.

The treatment of acetylcysteine toxicity is usually aimed at reversing anaphylactoid symptoms or controlling nausea and vomiting. The use of epinephrine, antihistamines, and steroids may be beneficial.
Hypersensitivity to Acetylcysteine or any component.
The sulfur odour on opening the sachets does not indicate deterioration of the product, but is due to the active ingredient.
Do not take product if the patient is hypersensitive to any of the ingredient.
Used with caution in asthmatic patients.
Used with caution in patients with a history of peptic ulcer disease.
Effects on ability to drive and use machine: None known.
Pregnancy risk factor B: use during pregnancy only if clearly needed.
Lactation: It is not known whether Acetylcysteine is excreted in human milk. Because many drugs are excreted in human milk, exercise caution when administering this drug to a nursing woman.
Oral administration of acetylcysteine, especially in large dose needed to treat paracetamol overdose, may result in nausea, vomiting, and other GI symptoms. Rash with or without mild fever has been observed rarely.
There are no known interactions where it is recommended to avoid concomitant use.
Incompatibilities: If Mucotic HD is administered concurrently with antibiotics, they should be administered 2 hours apart from Mucotic HD in order to avoid possible interaction.
