Sertraline
Indications/Uses
Depression, obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder.
Dosage/Direction for Use
Depression/OCD Adult Initially 50 mg/day. Childn & adolescent 13-17 yr Initially 50 mg once daily, 6-12 yr 25 mg once daily. Elderly Initially 25 mg/day in the morning, increase by 25 mg/day increments every 2-3 days if tolerated to 50-100 mg/day; may be increased up to max 200 mg/day.
Administration
May be taken with or without food.
Contraindications
Concomitant MAOI, pimozide; disulfiram (w/ high conc of sertraline).
Special Precautions
Do not use w/in 14 days of MAOI therapy. Preexisting seizure disorder, undesirable wt loss, recent MI, unstable heart disease, hepatic or renal impairment, concomitant psychotropic medications, agitated or hyperactive patients. Hyponatremia especially in elderly. Pregnancy, lactation.
Adverse Reactions
Insomnia, somnolence, dizziness, headache, fatigue, xerostomia, diarrhea, nausea, ejaculatory disturbances, palpitations, agitation, anxiety, nervousness, rash, decreased libido, constipation, anorexia, dyspepsia, flatulence, vomiting, wt gain, micturition disorders, tremors, paresthesia, visual difficulty, abnormal vision, tinnitus, increased diaphoresis.
Drug Interactions
Fatal reactions w/ MAOI, pimozide. Mania, HTN or serotonin syndrome w/ selegiline. SSRIs, amphetamine, buspirone, meperidine, nefazodone, serotonin agonists, sibutramine, sympathomimetics, ritonavir, tramadol, venlafaxine may increase risk of serotonin syndrome. May increase effects of benzodiazepines, carbamazepine, carvedilol, clozapine, cyclosporine, dextromethorphan, digoxin, haloperidol, HMG-CoA reductase inhibitors, phenytoin, propafenone, trazodone, TCA, valproic acid & phenothiazines. May increase nephrotoxicity w/ lithium. May increase risk of hyponatremia w/ loop diuretics. May increase hypoprothrombinemic response to warfarin. May decrease effect of tolbutamide.
