Fluticasone propionate
Indications/Uses
Relief of inflammatory & pruritic manifestations of corticosteroid-responsive dermatoses including atopic dermatitis, nummular dermatitis (discoid eczemas), prurigo nodularis, psoriasis (excluding widespread plaque psoriasis), lichen simplex chronicus (neurodermatitis) & lichen planus, seborrhoeic dermatitis, irritant or allergic contact dermatitis, discoid lupus erythematosus; adjunct to systemic steroid therapy in generalised erythroderma; insect bite reactions, miliaria (prickly heat). Reduction of the risk of relapse of chronic recurrent atopic dermatitis, once an acute episode has been treated effectively.
Dosage/Direction for Use
Adult, childn & infant ≥3 mth Treatment of inflammatory dermatoses Apply a thin layer & gently rub in using only enough to cover the entire affected area, once daily or bd for up to 4 wk until improvement occurs, then reduce frequency of application or change the treatment to a less potent prep. Allow adequate time for absorption after each application before applying an emollient. Atopic dermatitis Therapy w/ topical corticosteroids should be gradually discontinued once control is achieved & an emollient continued as maintenance therapy. Reduction of the risk of relapse Once an acute episode has been treated effectively, application frequency should be reduced to once daily application, twice wkly, w/o occlusion. This should be combined w/ routine daily use of emollients.
Contraindications
Rosacea; acne vulgaris; perioral dermatitis; untreated cutaneous infections; perianal & genital pruritus; pruritus w/o inflammation; dermatoses in infants <3 mth, including dermatitis & nappy rash.
Special Precautions
Caution in patients w/ history of local hypersensitivity to corticosteroids. Local hypersensitivity reactions may resemble symptoms of the condition under treatment. Manifestations of hypercortisolism & reversible hypothalamic-pituitary-adrenal axis suppression as a result of increased systemic absorption. Avoid long-term continuous therapy especially in infant & childn 3 mth to 12 yr. Avoid prolonged application to face & contact w/ eyes. Appropriate antimicrobial therapy should be used whenever treating inflammatory lesions which have become infected. W/draw if there is spread of infection. Infection risk w/ occlusion. Use w/ caution in psoriasis & chronic leg ulcers. Visual disturbance. Contains imidurea, cetostearyl alcohol & propylene glycol. Patients w/ renal/hepatic impairment (in case of systemic absorption). Pregnancy & lactation. Elderly.
Drug Interactions
Increased systemic exposure w/ CYP3A4 inhibitors (eg, ritonavir, itraconazole).
