Indications/Uses
Reduce LDL-cholesterol, apolipoprotein B & triglycerides; increase HDL-cholesterol in the treatment of hyperlipidaemias including hypercholesterolaemias & combined (mixed) hyperlipidaemia (type IIa or IIb hyperlipoproteinaemias), hypertriglyceridaemia (type IV) & dysbetalipoproteinaemia (type III). Adjunctive therapy in patients w/ homozygous familial hypercholesterolaemia who have some LDL-receptor function.
Dosage/Direction for Use
Primary hypercholesterolaemia (heterozygous familial or nonfamilial) & mixed dyslipidaemia Adult Initial dose: 10 or 20 mg once daily; 40 mg daily in patients requiring >45% reductions in LDL-cholesterol. Maintenance dose: 10-80 mg once daily. Heterozygous familial hypercholesterolaemia Childn ≥10 yr 10 mg once daily. Max: 20 mg daily. Homozygous familial hypercholesterolaemia 10-80 mg once daily. Adjust dose at intervals of no <4 wk until desired effect is observed.
Administration
May be taken with or without food: Avoid excessive consumption (>1 L/day) of grapefruit juice.
Contraindications
Hypersensitivity. Active liver disease; unexplained, persistent increases in serum aminotransferase conc. Pregnancy & lactation.
Special Precautions
Myogenic adverse reactions including myasthenia & myositis especially in patients w/ risk factors eg, >65 yr, uncontrolled hypothyroidism, renal disease. Control hypercholesterolemia w/ appropriate diet, exercise & wt reduction in obese patients before instituting therapy. Non-serious & reversible-upon-discontinuation cognitive impairment (eg, memory loss, forgetfulness, amnesia, memory impairment, confusion). Increases in HbA1c & fasting serum glucose levels. Persistent elevations in serum transaminases. Perform liver function tests prior to & 12 wk after initiation of therapy & periodically thereafter. Caution in patients who consume substantial quantities of alcohol &/or have a history of liver disease. Uncomplicated myalgia. Promptly report unexplained muscle pain, tenderness or weakness, particularly if accompanied by malaise or fever. Discontinue if markedly elevated creatine phosphokinase levels occur or myopathy is diagnosed or suspected. Childn <10 yr.
Adverse Reactions
Constipation, flatulence, dyspepsia, abdominal pain. 20 mg: Insomnia, headache, nausea, diarrhea, myalgia, asthenia.
Drug Interactions
Increased risk of myopathy w/ cyclosporine, fibric acid derivatives, niacin (nicotinic acid), erythromycin, azole antifungals. Decreased plasma conc but greater LDL-C reduction w/ colestipol. Increased plasma conc of digoxin. Increased plasma conc w/ erythromycin. Increased AUC values of norethindrone & ethinylestradiol. Increased plasma conc w/ strong CYP3A4 inhibitors. Increased risk of adverse effects w/ grapefruit juice.
