Telmisartan
Indications/Uses
Treatment of essential HTN. Reduction of CV morbidity in adults w/ manifest atherothrombotic CV disease (history of CHD, stroke or peripheral arterial disease) or type 2 DM w/ documented target organ damage.
Dosage/Direction for Use
Adult Treatment of essential HTN 40 mg once daily. Max: 80 mg once daily. Prevention of CV morbidity 80 mg once daily. Mild to moderate hepatic impairment Max: 40 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to telmisartan. Pregnancy (2nd & 3rd trimesters). Biliary obstruction, severe hepatic impairment, concomitant use w/ aliskiren-containing products in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2).
Special Precautions
Hepatic impairment. Renovascular HTN. Monitor K & creatinine levels in patients w/ impaired renal function. Intravascular hypovolaemia should be corrected prior to treatment. Dual blockage of RAAS. Severe CHF, primary aldosteronism, aortic or mitral valve stenosis, obstructive hypertrophic cardiomyopathy. Antidiabetics & insulin. Monitor serum K in patients at risk of hyperkalaemia. Rare hereditary condition of fructose intolerance. Ethnic differences. Childn <18 yr.
Adverse Reactions
UTI, upper resp tract infections, anaemia, hyperkalaemia, insomnia, depression, syncope, vertigo, bradycardia, hypotension, orthostatic hypotension, dyspnoea, cough, abdominal pain, diarrhoea, dyspepsia, flatulence, vomiting, hyperhidrosis, pruritus, rash, back & chest pain, muscle spasms, myalgia, renal impairment, asthenia, increased blood creatinine.
Drug Interactions
Digoxin, K supplements, K-containing salt substitutes, K-sparing diuretics, ACE inhibitors, AIIA, aliskiren, NSAIDs, heparin, lithium. May increase hypotensive effect of other antihypertensive agents. Immunosuppressives & trimethoprim. Increased hypotensive effects w/ baclofen & amifostine. Aggravated orthostatic hypotension w/ alcohol, barbiturates, narcotics or antidepressants. Systemic corticosteroids.
