Indications/Uses
Symptomatic relief of OA. Pain & signs of inflammation associated w/ acute gouty arthritis. Relief of chronic musculoskeletal pain; acute pain associated w/ dental surgery.
Dosage/Direction for Use
OA & chronic musculoskeletal pain 60 mg once daily. Acute gouty arthritis 120 mg once daily. Max duration of treatment: 8 days. Acute pain associated w/ dental surgery 90 mg once daily. Moderate hepatic insufficiency (Child-Pugh score7-9) Max: 60 mg every other day, or 30 mg once daily. Mild hepatic insufficiency (Child-Pugh score 5-6) Max: 60 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. CHF (NYHA II-IV), established ischemic heart disease, peripheral arterial disease &/or cerebrovascular disease (including patients who have recently undergone CABG surgery or angioplasty).
Special Precautions
Patients w/ known CV disease or risk factors for CV disease; HTN; fluid retention or heart failure; prior history of GI perforation, ulcers & bleeding (PUB) & patients >65 yr; medical history of ischemic heart disease; previously experienced acute asthmatic attacks, urticaria, or rhinitis. Closely monitor BP during the initiation of NSAID treatment & throughout the course of therapy. Concomitant use of oral corticosteroids or anticoagulants, longer duration of NSAID therapy, smoking, use of alcohol, older age & poor general health status. Periodically re-evaluate patients need for symptomatic relief & response to therapy. Patients w/ significant risk factors for CV events (eg, hyperlipidemia, DM, smoking) or peripheral arterial disease. Anti-platelet therapies should not be discontinued. Patients w/ considerable dehydration. Rehydrate patients prior to starting therapy. Consider the possibility of exacerbating fluid retention, edema or HTN in patients w/ pre-existing edema, HTN, or heart failure. May mask fever, which is a sign of infection. Pre-existing significantly impaired renal function, uncompensated heart failure, or cirrhosis. Not recommended in patients w/ advanced renal disease. Close monitoring of the patient's renal function is advisable. Elevated ALT &/or AST in patients treated for up to 1 yr. Evaluate for persistently abnormal liver dysfunction in patients w/ symptoms &/or signs suggesting liver dysfunction, or in whom an abnormal LFT has occurred. Discontinue if persistently abnormal LFTs (3x the ULN) are detected. First 2 trimesters of pregnancy. Avoid in late pregnancy; may cause premature closure of the ductus arteriosus. Lactation. Ped patients. Elderly or debilitated patients.
Adverse Reactions
Asthenia/fatigue, dizziness, lower extremity edema, HTN, dyspepsia, heartburn, nausea, headache, increased ALT & AST. Thrombocytopenia; hypersensitivity reactions including anaphylactic/anaphylactoid reactions eg, shock; hyperkalemia; anxiety, insomnia, confusion, hallucinations, depression, restlessness; dysgeusia, somnolence; blurred vision; CHF, palpitations, angina, arrhythmia; hypertensive crisis; bronchospasm; abdominal pain, oral ulcers, peptic ulcers including perforation & bleeding (mainly in elderly), vomiting, diarrhea; hepatitis, jaundice, hepatic failure; angioedema, pruritus, rash, SJS, TEN, urticaria, fixed drug eruption; renal insufficiency including renal failure.
Drug Interactions
Increased prothrombin time INR w/ warfarin. Decreased plasma AUC w/ rifampin. Increased plasma conc & reduced renal clearance of MTX. May diminish the antihypertensive effect of diuretics, ACE inhibitors & AIIAs. May increase plasma lithium levels. Increased rate of GI ulceration or other complications w/ low-dose aspirin. Increased steady state AUC0-24hr of OCs eg, ethinyl estradiol; HRT.
