Lamotrigine

Monotherapy or adjuvant therapy in epilepsy, partial & generalized seizures eg, tonic-clonic seizures associated w/ Lennox-Gastaut syndrome.

Epilepsy Adult >12 yr Monotherapy: Initially 25 mg once daily for 2 wk followed by 50 mg once daily for 2 wk, may be increased by a max of 50-100 mg every 1-2 wk. Maintenance: 100-200 mg once daily or in 2 divided doses. Adjunct to valproate: Initially 12.5 mg, given 25 mg every alternate day for 2 wk, may be increased by a max of 25-50 mg every 1-2 wk. Maintenance: 100-200 mg once daily or in 2 divided doses. Patient taking concomitant AEDs (except valproate) Initially 50 mg once daily for 2 wk, followed by 100 mg daily given in 2 divided doses for 2 wk, may be increased by max of 100 mg every 1-2 wk. Maintenance: 200-400 mg daily in 2 divided doses. Patient taking oxcarbazepine w/o any other inducers or inhibitors Initially 25 mg once daily for 2 wk, followed by 50 mg daily for 2 wk, may be increased by a max of 50-100 mg every 1-2 wk. Maintenance: 100-200 mg once daily or in 2 divided doses.

May be taken with or without food.

Hypersensitivity.

Adverse skin reaction may occur w/in the 1st 8 wk after treatment initiation. Discontinue in patients who develop drug-related rash. Monitor manifestations of hypersensitivity. Abrupt w/drawal may provoke rebound seizures; possibility of interference w/ folate metabolism during long-term therapy. Patients w/ renal failure. Pregnancy & lactation. Childn.

Skin rash, fever, lymphadenopathy. SJS, TEN. Blood & lymphatic system disorders. Irritability, aggression, tics, hallucination, confusion. Headache, drowsiness, insomnia, dizziness, tremor, ataxia. Diplopia, blurred vision, conjunctivitis. GI disorders. Increased LFT, hepatic dysfunction & failure. Lupus-like reactions. Tiredness.

Enhanced metabolism w/ antiepileptic drugs (eg, phenytoin, carbamazepine, phenobarbitone & primidone). Reduced metabolism w/ valproate.
