Escitalopram
Indications/Uses
Major depressive episodes, panic disorder w/ or w/o agoraphobia, social anxiety disorder (social phobia), generalized anxiety disorder, obsessive-compulsive disorder.
Dosage/Direction for Use
Major depressive episodes, social anxiety, generalized anxiety & obsessive-compulsive disorder 10 mg once daily. May be increased to a max of 20 mg daily. Panic disorder w/ or w/o agoraphobia Initially 5 mg daily for the 1st wk then increase to 10 mg daily. May be further increased to a max of 20 mg daily depending on individual patient's response. Elderly >65 yr Initially 5 mg daily. Patient w/ reduced hepatic function & poor metabolisers of CYP2C19 Initially 5 mg daily for the 1st 2 wk. May be increased to 10 mg daily depending on individual patient's response.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Concomitant use w/ non-selective, irreversible MAOIs; pimozide.
Special Precautions
CHD, paradoxical anxiety, seizures, history of mania or hypomania, diabetes, suicide/suicidal thoughts, akathisia/psychomotor restlessness, hyponatremia, hemorrhage. Electroconvulsive therapy. Patients w/ angle-closure glaucoma or a history of glaucoma. Abrupt w/drawal. Concomitant use w/ reversible selective MAO-A inhibitors, products w/ serotonergic effects (eg, sumatriptan or other tryptans, tramadol, tryptophan), St. John's wort. May impair the ability to drive the vehicle or operate the machinery. Pregnancy & lactation. Childn & adolescents <18 yr.
Adverse Reactions
Thrombocytopenia; anaphylaxis reactions; inappropriate ADH secretions; reduced or increased appetite, wt gain; reduced body wt; hyponatremia, anorexia; anxiety, restlessness, abnormal dream, reduced libido (in male), anorgasm (in woman); bruxism, agitation, nervousness, panic attack, mental confusion; aggression, depersonalization, hallucination; mania, suicide idea or actions; insomnia, drowsiness, dizziness, paresthesia, tremor; taste disturbances, sleep disturbances, syncope; serotonin syndrome, dyskinesia, movement disorders, convulsions, akathisia; mydriasis, visual disturbances, tinnitus; tachycardia; bradycardia; prolonged QT interval, orthostatic hypotension; sinusitis, yawning, epistaxis, nausea; diarrhea, constipation, vomiting, dry mouth; GI bleeding (including rectum bleeding); hepatitis, LFT abnormality; hyperhidrosis; urticaria, alopecia, pruritus, rash; ecchymosis, angioedema; arthralgia, myalgia; urinary retention. Male: Ejaculation disorders, impotence. Female: Metrorrhagia; galactorrhea.
Drug Interactions
Cases of serious reactions in patients receiving an SSRI w/ irreversible non-selective MAOI & recently discontinued SSRI treatment. Combination w/ irreversible non-selective MAOIs is contraindicated. Increase in AUC & Cmax of pimozide. Increased QTc interval w/ pimozide. Risk of serotonin syndrome w/ reversible, selective MAO-A inhibitor eg, moclobemide; irreversible MAO-B inhibitor eg, selegiline; serotonergic medicinal products eg, tramadol, sumatriptan & other triptans. Lowered seizure threshold w/ antidepressants (eg, tricyclics, other SSRIs), neuroleptics (eg, phenothiazines, thioxanthenes, butyrophenones), mefloquine, bupropion & tramadol. Enhanced effects w/ lithium or tryptophan. Increased incidence of AR w/ St. John's wort. May alter anticoagulant effects of oral anticoagulants. Concomitant use w/ alcohol. Increased plasma conc w/ omeprazole or cimetidine. Concomitant use w/ CYP2C19 inhibitors eg, omeprazole, fluoxetine, fluvoxamine, lansoprazole, ticlopidine, cimetidine; medicinal products metabolized by CYP2D6 & w/ narrow therapeutic index (eg, flecainide, propafenone or metaprolol) or by CYP2C9, some CNS-acting medicinal products eg, antidepressants (eg, desipramine, clomipramine & nortriptyline), antipsychotics (eg, risperidone, thioridazine, haloperidol).
