Tramadol
Indications/Uses
Moderate to severe acute & chronic pain & in painful diagnostic or therapeutic measures. Post-op pain & labour pain. Musculoskeletal pain. Cancer pain. Neuropathic pain.
Dosage/Direction for Use
Cap Adult & childn >14 yr 1 cap, up to 8 cap daily. Supp Adult & childn >14 yr 1 supp, up to 4 supp daily. 50 mg/mL amp 1-2 amp IV, IM or SC, up to 8 amp daily. 100 mg/2 mL amp 1 amp IV, IM or SC, up to 8 amp daily.
Administration
May be taken with or without food.
Contraindications
Acute intoxication w/ alcohol, hypnotics, analgesics or psychotropics. Narcotic w/drawal treatment.
Special Precautions
Opioid dependence, reduced level of consciousness of unclear origin, resp disorders, increased ICP. Patients known to suffer from convulsions. On long-term use, possibility of tolerance, psychic & physical dependence. May impair ability to drive or operate machinery, especially if taken w/ alcohol. Pregnancy & lactation.
Adverse Reactions
Sweating, dizziness, muzziness, vomiting, dry mouth. In rare cases, influence on CVS regulation, especially after IV administration, headache, retching, vomiting, constipation, GI irritation, skin reactions. In very rare cases, motorial weakness, blurred vision, appetite changes, micturition disorders, psychic side effects eg, mood, perception & activity changes, allergic reactions & anaphylaxis. Cerebral convulsions especially on co-medication w/ neuroleptics, increase in BP, bradycardia. If recommended dosage is exceeded considerably, possibility of resp depression cannot be excluded.
Drug Interactions
Avoid concurrent administration of MAOIs. Co-medication w/ other centrally-acting depressants including alcohol may potentiate CNS effects. Carbamazepine may reduce analgesic effect & duration of action. Co-medication w/ seizure threshold-lowering drugs (eg, SSRIs, TCAs, antipsychotics) may cause convulsions.
