Indications/Uses
Dosage/Direction for Use
300-600 mg daily in 2-3 divided doses. Severe depression Initially 300 mg daily, may be increased to 600 mg daily.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Acute confusional states. Co-administration w/ selegiline, triptans, pethidine, tramadol. bupropion, dextromethorphan, linezolid. Childn.
Special Precautions
Hypersensitivity. Monitor for clinical worsening, suicidal behaviour or thoughts & unusual changes in behaviour. Depressive patients w/ schizophrenic or schizoaffective psychoses, history of suicide-related events or suicidal ideation. Interrupt treatment if early symptoms of or mania or hypomania occur. Thyrotoxicosis, phaeochromocytoma. Avoid large consumption of tyramine-rich food. Not to be taken by patients w/ galactose intolerance, total lactase deficiency or glucose-galactose malabsorption. Avoid concomitant administration w/ clomipramine. Not recommended in concomitant administration w/ dextromethorphan. Co-administration w/ multi-drug antidepressants; buprenorphine-containing products; serotonergic agents; St. John's wort-containing herbal products. May affect ability to drive & use machines. Hepatic disease. Pregnancy & lactation. Childn & adolescents.
Adverse Reactions
Sleep disorders; dizziness, headache; dry mouth, nausea. Agitation, anxiety, restlessness; paraesthesia; hypotension; vomiting, diarrhoea, constipation; rash; irritability.
Drug Interactions
Increased plasma conc of triptans eg, sumatriptan, rizatriptan, zolmitriptan, almotriptan, naratriptan, frovatriptan, eletriptan; PPIs (eg, omeprazole), fluoxetine, fluvoxamine; trimipramine, maprotiline. Potentiated effects of opiates. Serotonin syndrome w/ opioids/opiate products eg, buprenorphine, buprenorphine-containing products; pethidine. Intensified & prolonged action of sympathomimetic agents. Increased CNS serotonin serum conc w/ St. John's wort products. Increased effects w/ sibutramine. Potentiated effects of dextropropoxyphene. Not to be co-administered w/ selegiline, linezolid, tramadol, dextromethorphan. Dose adjustment w/ morphine, fentanyl, codeine; cimetidine. Minor potentiated pressor effect w/ tyramine. Caution w/ antidepressants eg, venlafaxine, clomipramine, citalopram, escitalopram, paroxetine, sertraline, bupropion; TCAs, other antidepressants.
