Folinic acid
Indications/Uses
Diminish toxicity & counteract action of folic acid antagonist eg, MTX in cytotoxic therapy & overdose in adult & childn (Ca folinate rescue). In combination w/ 5-fluorouracil for metastasized colorectal carcinoma.
Dosage/Direction for Use
Ca folinate rescue Folic acid antagonists eg, MTX: Up to 120 mg given over 12-24 hr by IM or IV inj or infusion, followed by 12-15 mg IM or 15 mg orally 6 hrly for next 48 hr. Megaloblastic anaemia 1 tab daily. Adult 10-20 mg daily. Childn up to 12 yr 0.25 mg/kg daily.
Contraindications
Hypersensitivity to folinate. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption.
Special Precautions
Maintenance of urine output. Measurement of plasma MTX conc, BUN & creatinine on days 2, 3 & 4. Concomitant use w/ folic acid antagonists. Pregnancy & lactation.
Adverse Reactions
Anaphylactoid reactions & urticaria.
Drug Interactions
Counteracted antiepileptic effect of phenobarb, phenytoin & primidone. Increased frequency of seizures in susceptible patients. Associated w/ seizures & syncope in concurrent fluoropyrimidine.
