Pantoprazole Na sesquihydrate
Indications/Uses
Short-term use for symptomatic improvement & healing of duodenal & gastric ulcer, moderate & severe reflux oesophagitis, Zollinger-Ellison syndrome & other pathological hypersecretory conditions.
Dosage/Direction for Use
Gastric & duodenal ulcer, moderate & severe reflux esophagitis 40 mg IV daily. Long-term management of Zollinger-Ellison syndrome & other pathological hypersecretory conditions Initially 80 mg IV daily, may be titrated up or down as needed. Doses >80 mg IV daily to be given bd. Rapid acid control Initially 2 x 80 mg IV. Severe hepatic impairment Not to exceed 20 mg IV daily.
Contraindications
Hypersensitivity to pantoprazole or substituted benzimidazoles.
Special Precautions
Discontinue use if subacute cutaneous lupus erythematosus occurs. Exclude malignancy in presence of alarming symptom eg, significant unintentional wt loss, recurrent vomiting, dysphagia, haematemesis, anaemia or melaena. Increased risk of GI infections caused by bacteria eg, Salmonella, Campylobacter; hip, wrist & spine fracture; Clostridium difficile-associated diarrhea. Severe hypomagnesaemia; vit B12 malabsorption due to hypo- or achlorhydria. Discontinue use 5 days prior to chromogranin A measurements. Ensure adequate intake of vit D & Ca. Long-term & high-dose use. Not recommended to be co-administered w/ atazanavir. Concomitant use w/ digoxin or drugs causing hypomagnesaemia eg, diuretics. May affect ability to drive & use machines. Severe hepatic impairment. Not to be used during pregnancy. Lactation.
Adverse Reactions
Benign fundic gland polyps; inj site thrombophlebitis.
Drug Interactions
Reduced absorption of drugs w/ gastric pH dependent bioavailability eg, some azole antifungals (eg, ketoconazole, itraconazole, posaconazole), erlotinib. Substantial reduction in bioavailability of HIV medications. Monitor prothrombin time/INR w/ coumarin anticoagulants eg, phenprocoumon or warfarin. Increased systemic exposure w/ CYP2C19 inhibitors eg, fluvoxamine. Reduced plasma conc w/ CYP2C19 & CYP3A4 inducers eg, rifampicin, St. John's wort (Hypericum perforatum). Increased MTX levels.
