Sertraline HCl
Indications/Uses
Symptomatic treatment of depression including depression accompanied by anxiety symptoms in patients w/ or w/o history of mania. Obsessive compulsive disorders (OCD); panic disorder w/ or w/o agoraphobia; PTSD; social phobia (social anxiety disorder).
Dosage/Direction for Use
Depression, OCD 50 mg daily. Panic disorder, PTSD & social phobia Initially 25 mg daily, increase dose to 50 mg once daily after 1 wk. Dose changes should be made at intervals of at least 1 wk up to max: 200 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Concomitant use of MAOIs, pimozide.
Special Precautions
Monitor for emergence of signs & symptoms of serotonin syndrome or NMS, suicide/suicidal thoughts or clinical worsening. Risk factors for QTc prolongation; activation of mania/hypomania; seizures; sexual dysfunction; abnormal bleeding/hemorrhage, history of bleeding disorders; hyponatremia; bone fractures; diabetes or loss of glycemic control; angle-closure glaucoma or history of glaucoma. Switching from antidepressants or anti-obsessional drugs. False +ve urine immunoassay screening tests for benzodiazepines. Not to be used in combination w/ MAOI or w/in 14 days of discontinuation. Avoid use w/ other drugs enhancing effects of serotonergic neurotransmission. Co-administration w/ SSRIs, drugs affecting platelet function. May affect ability to drive & use machines. Hepatic impairment. Women of childbearing potential must use effective contraception. Pregnancy. Not recommended during lactation. Monitor growth & development during long-term use in childn & adolescents.
Adverse Reactions
Thrombocytopenia, leukopenia; anaphylactoid reaction, hypersensitivity; SIADH, hyperprolactinaemia, hypothyroidism; DM, hyponatraemia, hypoglycaemia, hyperglycaemia, decreased/increased appetite; psychotic disorder, hallucination, aggression, confusional state, euphoric mood, depressive symptom, anxiety, agitation, bruxism, nightmare, decreased libido, insomnia; serotonin syndrome, coma, convulsion, dystonia, akathisia, syncope, extrapyramidal disorder, involuntary muscle contractions, hypoaesthesia, hyperkinesia, migraine, hypertonia, tremor, somnolence, paraesthesia, dizziness, headache; visual impairment, mydriasis, periorbital oedema; tinnitus; Torsade de pointes, palpitations, tachycardia; vascular, resp, thoracic & mediastinal, GI, skin & SC tissue disorders; liver injury; rhabdomyolysis, trismus, muscle spasms, arthralgia; urinary retention & incontinence, haematuria, enuresis; priapism, galactorrhoea, gynaecomastia, ejaculation disorder, sexual dysfunction, irregular menstruation; face oedema, drug w/drawal syndrome, gait disturbance, peripheral oedema, chest pain, malaise, pyrexia, asthenia, fatigue; decreased/increased wt, abnormal lab & platelet function tests, ECG prolonged QT, increased blood cholesterol, ALT & AST; fracture.
Drug Interactions
Increased pimozide levels. Increased risk of QTc prolongation &/or ventricular arrhythmia w/ drugs prolonging QTc intervals. Increased in tremor w/ lithium. Reduced plasma levels w/ phenytoin. Monitor prothrombin time w/ warfarin. Changes in pharmacokinetic parameters w/ diazepam, tolbutamide. Decreased clearance w/ cimetidine. Decreased clinical efficacy w/ metamizole. Concomitant use w/ sumatriptan. Not recommended in concomitant use w/ alcohol.
