Amphotericin B (lipid complex)
Indications/Uses
Invasive fungal infection in patients who are refractory to or intolerant of conventional amphotericin B therapy. Severe systemic fungal infections including aspergillosis, blastomycosis, candidiasis, coccidioidomycosis, cryptococcosis, histoplasmosis, mucormycosis, paracoccidioidomycosis & sporotrichosis. Treatment of choice in fungal endocarditis, meningitis, peritonitis or severe resp tract infections.
Dosage/Direction for Use
Severe systemic infections 5 mg/kg by IV infusion at a rate of 2.5 mg/kg/hr for at least 14 days. Initial test dose: 1 mg infused IV over 15 min.
Contraindications
Special Precautions
Not to be used for treating common or superficial inapparent fungal infections (detectable only by +ve skin or serologic tests). Monitor renal function prior to & at least once wkly during therapy. Patients undergoing renal dialysis (administer only after completion of dialysis). Monitor serum K & Mg levels regularly. May increase serum bilirubin, alkaline phosphatase or transaminase. Pregnancy & lactation.
Adverse Reactions
Chills, fever, nausea & vomiting. Increased serum creatinine, azotemia & hypokalemia.
Drug Interactions
Close monitoring of renal function is required w/ nephrotoxic drugs. Exacerbated myelotoxicity & nephrotoxicity w/ zidovudine. Increased serum creatinine w/ cyclosporin. Antineoplastics, corticosteroids, ACTH, digitalis glycosides, skeletal muscle relaxants.
