HTN 50 mg once daily, may be increased to 100 mg as a single dose or in 2 divided doses. Hypertensive patient w/ left ventricular hypertrophy Initially 50 mg once daily. Low dose hydrochlorothiazide may be added or may increase dose to 100 mg once daily. Nephropathy in type 2 diabetic patient Initially 50 mg once daily, may be increased to 100 mg once daily. Elderly >75 yr & patient w/ moderate to severe renal impairment (CrCl <20 mL/min) or intravascular fluid depletion Initially 25 mg once daily.
Reduced levels of active metabolite w/ rifampin & fluconazole. Increase serum K w/ K-sparing diuretics (eg, spironolactone, triamterene, amiloride), K supplements or K-containing salt substitutes. May be attenuated the antihypertensive effect w/ NSAID indomethacin.
