Azithromycin dihydrate
Indications/Uses
Acute bacterial sinusitis & otitis media; pharyngitis, tonsillitis; acute exacerbation of chronic bronchitis; mild to moderately severe community acquired pneumonia; skin & soft tissue infections; uncomplicated Chlamydia trachomatis urethritis & cervicitis.
Dosage/Direction for Use
Adult Uncomplicated Chlamydia trachomatis urethritis & cervicitis 1,000 mg as single dose. All other indications 500 mg daily for 3 consecutive days (total of 1,500 mg). Alternatively, may give 500 mg on the 1st day & 250 mg on the 2nd-5th day (total of 1,500 mg). Elderly Same as adult dose. Childn >45 kg Use normal adult dose, <45 kg May use other pharmaceutical forms eg, susp.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to azithromycin dihydrate, erythromycin, any macrolide or ketolide antibiotic, or lactose. Patients w/ history of cholestatic jaundice/hepatic dysfunction associated w/ prior use of azithromycin.
Special Precautions
Serious allergic & skin reaction. Discontinue therapy if reaction or hepatitis occurs. Hepatotoxicity. QT prolongation or history of torsades de pointes, those w/ pro arrhythmic conditions & w/ other drugs that prolong QT interval. Clostridium difficile-associated diarrhea. Evaluate patients if diarrhea occurs. May exacerbate muscle weakness in patients w/ myasthenia gravis. Mild to moderate renal (GFR 10-80 mL/min) & liver impairment.
Adverse Reactions
Transient episodes of mild neutropenia; hearing impairment; nausea, vomiting, diarrhea, loose stools, abdominal discomfort (pain/cramps) & flatulence; abnormal liver function; allergic reactions including rash & angioedema; local pain & inflammation at the site of infusion.
Drug Interactions
Co-administration w/ antacids, efavirenz, fluconazole, nelfinavir, rifabutin, terfenadine, cimetidine.
