Indications/Uses
Duodenal ulcer; gastric ulcer; reflux esophagitis & esophageal ulceration; Zollinger-Ellison syndrome; resistant ulcer; eradication of H. pylori in combination w/ antibiotics.
Dosage/Direction for Use
IV over 5 min or IV infusion over 20-30 min. Prophylaxis of acid aspiration 40 mg completed 1 hr before surgery. Benign gastric ulcer, duodenal ulcer & gastroesophageal reflux 40 mg once daily until PO. Possible severe peptic ulcer bleeding (unlicensed indication) Initially 80 mg by IV infusion, then 8 mg/hr by continuous IV infusion for 72 hr. Patient unsuited to receive oral therapy 40 mg of the base over a period of 20-30 min on a short-term basis by IV infusion. May also be given by slow IV inj. Zollinger-Ellison syndrome Given higher IV doses. Childn 1 mth-12 yr 500 mcg/kg (max of 20 mg) once daily. May be increased to 2 mg/kg (max of 40 mg) once daily if needed.
Contraindications
Hypersensitivity to omeprazole & other substituted benzimidazoles (rabeprazole, esomeprazole).
Special Precautions
Atrophic gastritis in long-term treatment. Gastric malignancy. Patients w/ various degrees/types of hepatic disease. Pregnancy & lactation. Childn <2 yr. Elderly.
Adverse Reactions
Headache, diarrhoea & skin rashes; pruritus, dizziness, fatigue, constipation, nausea & vomiting, flatulence, abdominal pain, arthralgia & myalgia, urticaria & dry mouth; hypersensitivity reactions; insomnia, somnolence & vertigo; reversible confusional states, agitation, depression & hallucinations; raised liver enzymes; increased risk of GI infections.
Drug Interactions
May affect azole antifungal agents (eg, itraconazole, ketoconazole), benzodiazepines, cilostazol, clarithromycin, digoxin, phenytoin, salicylates, sulfonylureas & warfarin; cyclosporine, disulfiram, azole antifungal agents, hydantoins. May interfere absorption of drugs where gastric pH is an important determinant of bioavailability (eg, ketoconazole, ampicillin, Fe salts, digoxin, cyanocobalamin). Drugs that affect PPIs include clarithromycin & sucralfate.
