Per g Calcipotriol (Oint: hydrate; Gel: monohydrate) 50 mcg, betamethasone dipropionate 500 mcg
Indications/Uses
Oint: Management of psoriasis vulgaris. Gel: Topical treatment of scalp psoriasis & mild to moderate non-scalp plaque psoriasis vulgaris.
Dosage/Direction for Use
Apply to the affected area once daily. Max daily dose: 15 g. Recommended treatment period: Oint 4 wk. Gel 4 wk for scalp areas & 8 wk for non-scalp areas. Max wkly dose: 100 g. Max treated area: 30% of body surface. Gel Affected scalp areas Between 1 & 4 g daily.
Contraindications
Hypersensitivity. Patients w/ known disorders of Ca metabolism. Viral (eg, herpes or varicella) lesions of the skin, fungal or bacterial skin & parasitic infections, skin manifestations in relation to TB, perioral dermatitis, atrophic skin, striae atrophicae, fragility of skin veins, ichthyosis, acne vulgaris/rosacea, rosacea, ulcers & wounds. Erythrodermic, exfoliative & pustular psoriasis.
Special Precautions
Adrenocortical suppression or impact on the metabolic control of DM may occur. Avoid application under occlusive dressings, & on large areas of damaged skin, or on mucous membranes or in skin folds. Hypercalcaemia may occur if max daily dose (100 g for oint & 15 g for gel) is exceeded. Do not use on the sensitive areas (skin of the face & genitals) & wash hands after each application to avoid accidental transfer to these areas. Treat w/ antimicrobial therapy when lesions become secondarily infected & stop treatment if infection worsens. Risk of generalised pustular psoriasis or of rebound effects when discontinuing treatment. Increased risk of local & systemic effects w/ long-term use. Limit or avoid excessive exposure to either natural or artificial sunlight. May cause local skin reactions (eg, contact dermatitis), or irritation to the eyes & mucous membranes w/ butylhydroxytoluene content. Avoid concurrent use w/ other steroids. Combination w/ other anti-psoriatic products. Severe renal insufficiency or severe hepatic disorders. Pregnancy & lactation. Childn <18 yr.
Adverse Reactions
Pruritus. Calcipotriol: Application site reactions, skin irritation, burning & stinging sensation, dry skin, erythema, rash, dermatitis, eczema, aggravated psoriasis, photosensitivity & hypersensitivity reactions. Betamethasone dipropionate: Skin atrophy, telangiectasia, striae, folliculitis, hypertrichosis, perioral dermatitis, allergic contact dermatitis, depigmentation & colloid milia; generalised pustular psoriasis; adrenocortical suppression, cataract, infections & increase IOP. Oint: Skin exfoliation.
