Dobutamine HCl
Indications/Uses
Inotropic support of the myocardium in the treatment of acute CHF or cardiogenic shock.
Dosage/Direction for Use
IV infusion of 2.5-40 mcg/kg/min. Usual dose: 2.5-10 mcg/kg/min.
Contraindications
Hypersensitivity. Patients w/ idiopathic hypertrophic subaortic stenosis.
Special Precautions
Continuously monitor ECG & BP during treatment; cardiac filling pressure & output; hypovolaemia. May provoke chest pain in patients w/ CHD; ventricular ectopic activity. Patients w/ arterial HTN; excessive tachycardia may occur. Increases atrioventricular conduction.
Adverse Reactions
Dose-related increase in heart rate & BP; ectopic beats, angina or chest pain, palpitations. Rarely, ventricular tachycardia; cardiac rupture during stress testing.
Drug Interactions
Increase cardiac output & reduces pulmonary capillary wedge pressure w/ nitroprusside. Ineffective in patients recently treated w/ β-adrenoreceptor blocking drugs. May increase peripheral vascular resistance.
