Indications/Uses
1st-line treatment of patients w/ inoperable, locally advanced (stage IIIA or IIB) or metastatic (stage IV) non-small cell lung cancer in combination w/ cisplatin; locally advanced (nonresectable stage II or stage III) or metastatic (stage IV) adenocarcinoma of the pancreas. 1st-line treatment in metastatic breast cancer after failure of prior anthracycline-containing adjuvant chemotherapy in combination w/ paclitaxel. Bladder cancer.
Dosage/Direction for Use
IV infusion Adult Nonsmall cell lung cancer Monotherapy: 1,000 mg/m2 over 30 min once wkly for up to 3 wk, followed by 1 wk rest period. Combination therapy: Gemcitabine (3- or 4-wk schedule) + cisplatin 100 mg/m2 IV on day 1. 4-wk schedule: 1,000 mg/m2 over 30 min on days 1, 8 & 15 of each 28-day cycle; 3-wk schedule: 1,250 mg/m2 over 30 min on days 1 & 8 of each 21-day cycle. Pancreatic cancer 1,000 mg/m2 over 30 min once wkly for up to 7 wk, followed by 1 wk rest period. Subsequent cycle: Once wkly for 3 consecutive wk out of 4 wk. Breast cancer Paclitaxel 175 mg/m2 by 3-hr on day 1 + gemcitabine 1,250 mg/m2 over 30 min on days 1 & 8 of each 21-day cycle. Bladder cancer 1,000 mg/m2 over 30 min on days 1, 8 & 15 of each 28-day cycle (followed by 1 wk rest period) + cisplatin 70 mg/m2 on day 1 or 2 of each 28-day cycle.
Special Precautions
Prolonged infusion time >60 min or increased dosing frequency. Monitor for myelosuppression. Patients w/ preexisting renal impairment or hepatic insufficiency. Concurrent liver metastases or preexisting medical history of hepatitis, alcoholism or liver cirrhosis. May impair ability to drive or operate machinery. Pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Anemia, leukopenia, thrombocytopenia; petechiae, mild blood loss; serious hepatotoxicity; nausea, vomiting, diarrhea, stomatitis; dyspnea, bronchospasm, pulmonary toxicity; mild proteinuria, hematuria, hemolytic uremic syndrome (HUS), elevation of bilirubin or LDH, reticulocytosis, elevation of serum creatinine or BUN; bronchospasm; MI, cerebrovascular accident, arrythmia, HTN; flu syndrome, fever, asthenia; infections; hair loss; edema, peripheral edema; mild paresthesias.
