Paracetamol 500 mg, anhydrous caffeine 30 mg, phenylephrine HCl 10 mg, chlorpheniramine maleate 2 mg
Indications/Uses
Symptomatic relief from symptoms of myalgia & fever associated w/ common cold eg, running nose, sneezing, nasal congestion, blocked nose, rhinitis, sinusitis, headache, sore throat, eye & muscle pain, cough.
Dosage/Direction for Use
Adult, elderly, childn ≥12 yr 1 tab tid-qid. Max: 4 tab/day. Doses should not be repeated more frequently than every 4 hr. Do not take continuously for >7 days (w/o medical advice).
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Pheochromocytoma, closed angle glaucoma; HTN, hyperthyroidism, diabetes, heart disease. Concomitant use of other sympathomimetic decongestants; TCAs; β-blockers; MAOIs taken w/in the last 2 wk. Hepatic or severe renal impairment. Pregnancy & lactation. Childn <12 yr.
Special Precautions
Dose should not exceed or not be used for >5 days. Hyperplasia of prostate gland, thyroid gland diseases & in patients >70 yr w/ CV diseases. Concomitant use w/ alcohol, coffee, strong tea & other tonic drinks. Development of psychological dependence in long-term use. May cause depression of CNS w/ sudden discontinuation. May induce false +ve doping tests. May cause drowsiness, dizziness, blurred vision & psychomotor impairment which can influence ability to drive or operate any other machines.
Adverse Reactions
Paracetamol: Thrombocytopenia, agranulocytosis; anaphylaxis, cutaneous hypersensitivity reactions; bronchospasm, hepatic dysfunction. Caffeine anhydrous: Nervousness, anxiety, irritability, restlessness, excitability. Phenylephrine HCl: Nervousness; headache, dizziness, insomnia; increased BP; nausea, vomiting. Chlorphenamine maleate: Nervous system & cardiac disorders; allergic reactions; urinary retention; nausea, vomiting, diarrhea.
Drug Interactions
Increased risk of ergotism w/ ergotamine & methysergide. Enhanced sedative effects w/ hypnotics or anxiolytics. May inhibit metabolism of phenytoin. Phenylephrine HCl: Hypertensive interactions w/ MAOIs (including moclobemide). Increased risk of CV adverse effects w/ sympathomimetic amines & TCAs (eg, amitriptyline). Reduced efficacy of β-blockers & antihypertensives. Increased risk of irregular heartbeat or heart attack w/ digoxin & cardiac glycosides. Paracetamol: Increased absorption w/ metoclopramide or domperidone. Reduced absorption w/ cholestyramine. Enhanced anticoagulant effects of warfarin & other coumarins.
