Indications/Uses
Benign gastric & duodenal ulcers including NSAID-induced gastric & duodenal ulcers or gastroduodenal erosions, GERD, acid-related dyspepsia or disorders associated w/ hypersecretion of gastric acid (eg, Zollinger-Ellison syndrome). In combination w/ antibiotics as triple therapy for eradication of H. pylori in PUD.
Dosage/Direction for Use
Benign gastric & duodenal ulcer including NSAID-induced gastric & duodenal ulcer or gastroduodenal erosion 20 mg once daily for 4 wk in duodenal ulceration or 8 wk in gastric ulceration. May be increased to 40 mg daily in severe case. GERD 20 mg once daily for 4 wk, followed by further 4-8 wk if not fully healed. Patient w/ reflux oesophagitis refractory to other therapy 40 mg daily for 8 wk, can be continued at 20 mg once daily. Acid-related dyspepsia 20 mg once daily for 2-4 wk. Zollinger-Ellison syndrome Initially 60 mg once daily. Usual range: 20-120 mg daily (>80 mg in 2 divided doses); larger dose up to 180 mg daily in divided doses may be used. H. pylori eradication in PUD 40 mg once daily. Triple therapy regimen: Omeprazole 20 mg bid w/ 1 of the following combinations: Amoxicillin 500 mg & metronidazole 400 mg both tid for 1 wk or clarithromycin 250 mg & metronidazole 400 mg (or tinidazole 500 mg) both bid for 1 wk or amoxicillin 1 g & clarithromycin 500 mg both bid for 1 wk.
Contraindications
Special Precautions
Exclude possibility of gastric malignancy when gastric ulcer is suspected prior to treatment. Avoid during pregnancy. Lactation. Ped.
Adverse Reactions
Nausea, diarrhea, abdominal colic, paresthesia, dizziness & headache.
Drug Interactions
Drugs metabolized by cytochrome P450 system eg, cyclosporine, disulfiram, benzodiazepines. Delayed elimination of diazepam, phenytoin & warfarin.
