Indications/Uses
Short-term management of moderate to severe acute pain that requires analgesia at the opioid level.
Dosage/Direction for Use
Tab 10 mg every 4-6 hr. Inj Adult Initially 10 mg followed by 10-30 mg every 4-6 hr. May be given as often as every 2 hr if needed. Max: 90 mg daily. Elderly, renal impairment, patient weighing <50 kg Max: 60 mg daily. Max duration: 2 days. Childn >2 yr 0.5-1 mg /kg followed by 0.5 mg/kg every 6 hr.
Contraindications
Hypersensitivity to ketorolac tromethamine or other NSAIDs & patients in whom aspirin or other prostaglandin synthesis inhibitors induce allergic reactions. Active or history of GI bleeding or perforation, related to previous NSAIDs therapy. Active or history of recurrent peptic ulcer/hemorrhage (>2 distinct episodes of proven ulceration or bleeding). Severe heart failure. Moderate or severe renal impairment or in patients at risk of renal failure due to vol depiction or dehydration. Labour & delivery. Prophylactic analgesic before surgery due to inhibition of platelet aggregation & intraoperatively. Suspected or confirmed cerebrovascular bleeding patients who have had operations w/ a high risk of hemorrhage or incomplete hemostasis & those at high risk of bleeding. Concomitant use w/ ASA or other NSAIDs.
Special Precautions
Not to be given in patients w/ peptic ulcerations. Patients w/ infections; asthma or allergic disorders; hemorrhagic disorders, HTN. Renal, hepatic or cardiac function impairment. Monitor for the development of blood, kidney, liver or eye disorders. Pregnancy. Elderly.
Adverse Reactions
Nausea, dyspepsia, GI pain & bleeding; abdominal discomfort, hematemesis, gastritis, esophagitis, diarrhea, eructation, constipation, flatulence, fullness, melena, peptic ulcer, nonpeptic GI ulceration, rectal bleeding, ulcerative stomatitis. vomiting, hemorrhage, perforation pancreatitis; anxiety, drowsiness, dizziness, headache, sweating, dry mouth, nervousness, paresthesia, functional disorders, abnormal thinking, depression, euphoria, convulsions, excessive thirst, inability to concentrate, insomnia, malaise, fatigue, stimulation, vertigo, abnormal taste & vision, optic neuritis, myalgia, abnormal dreams, hallucinations, hyperkinesias, hearing loss, tinnitus, aseptic meningitis, psychotic reactions, nephrotoxicity including urinary frequency, oliguria, acute renal failure, hyponatremia, hyperkalemia, hemolytic uremic syndrome, flank pain (w/ or w/o hematuria), raised serum urea & creatinine, interstitial nephritis, urinary retention, nephritic syndrome; flushing bradycardia, pallor, purpura, thrombocytopenia, neutropenia, agranulocytosis, aplastic or hemolytic anemia, HTN, palpitations, chest pain, dyspnea, asthma, pulmonary edema; pruritus, urticaria, skin photosensitivity, Lyell's syndrome, SJS, exfoliative dermatitis, maculopapular rash; anaphylaxis, bronchospasm, laryngeal edema, hypotension, flushing & rash.
Drug Interactions
Enhanced effects of anticoagulants eg, warfarin. Increased risk of GI bleeding w/ antiplatelet agents & SSRIs. Increased risk of inducing serious NSAID-related adverse events w/ ASA or other NSAIDs. Increased tendency of bleeding w/ oxpentifylline. Decreased plasma clearance & vol of distribution, increased plasma conc & t1/2 w/ probenecid. Reduced clearance & possibly enhanced toxicity of methotrexate. Increase plasma conc of lithium. Reduced diuretic response to furosemide. Reduced effect of diuretics & antihypertensive drugs. Risk of acute renal insufficiency w/ ACE inhibitors &/or angiotensin II receptor antagonists. Decreased peak & delayed time-to-peak conc w/ high-fat meal.
