Levofloxacin (IV infusion: hemihydrate)
Indications/Uses
Treatment of mild, moderate & severe infections caused by susceptible strains of designated microorganisms in conditions eg, acute maxillary sinusitis, acute bacterial exacerbations of chronic bronchitis, community-acquired & nosocomial pneumonia, typhoid & paratyphoid fever, complicated skin & skin structure infections, UTI & acute pyelonephritis. IV infusion Used in chronic bacterial prostatitis, skin & soft tissue infections, complicated UTI & community-acquired pneumonia.
Dosage/Direction for Use
Tab Acute maxillary sinusitis 500 mg once daily for 10-14 days. Acute bacterial exacerbation of chronic bronchitis 500 mg once daily for 7 days. Community-acquired pneumonia 500 mg once daily for 7-14 days or 750 mg once daily for 5 days. Nosocomial pneumonia 750 mg once daily for 7-14 days. Typhoid & paratyphoid fever 500 mg once daily for 10-14 days. Uncomplicated skin & soft tissue infections 500 mg once daily for 7-10 days. Complicated UTI 750 mg once daily for 5 days. Chronic bacterial prostatitis 500 mg once daily for 28 days. IV infusion Community-acquired pneumonia 500 mg once daily for 7-14 days or 750 mg/day for 5 days. Nosocomial pneumonia 750 mg once daily for 7-14 days. Uncomplicated skin & soft tissue infections 500 mg once daily. Chronic bacterial prostatitis 500 mg once daily. Administer by slow IV infusion over 60 min for 500 mg & over 90 min for 750 mg.
Administration
May be taken with or without food: May be taken during or between meals. Swallow whole, do not crush. Take at least 2 hr before or 2 hr after antacids containing Mg/Al, sucralfate, metal cations (eg, Fe), & multivit prep w/ Zn.
Contraindications
Hypersensitivity to other quinolones. History of tendon disorders to fluoroquinolone therapy. Lactation. Childn & adolescents.
Special Precautions
Adequate hydration. Renal insufficiency. Patients w/ known or suspected CNS disorder that may predispose seizure or lower the seizure threshold. Periodic assessment of organ system functions including renal, hepatic & hematopoietic. Discontinue use if patient experience pain, inflammation or rupture of a tendon. Avoid excessive exposure to sunlight. Increased risk of aortic aneurysm & dissection; reserve use only when there are no alternative antibacterial treatments available. Pregnancy.
Adverse Reactions
Urticaria, cramping of bronchi, severe breathing problems. Itching & rash. Nausea & diarrhea, loss of appetite, vomiting, abdominal pain, dyspepsia, bloody diarrhea. Headache, vertigo or dizziness, drowsiness, sleeping problems, paresthesia. Abnormally rapid beating of the heart, hypotension & circulatory (shock-like) collapse. Tendon pain. Tendon rupture (Achilles tendon). Muscular weakness. Increased AST & ALT. Increased bilirubin & serum creatinine, inflammation of the liver, disturbance of kidney function up to kidney failure. Eosinophilia & leukopenia. Nightmares, visual & sensory disturbances.
Drug Interactions
Antacids, sucralfate, metal cations, multivit. NSAIDs. Antidiabetic drugs eg, glyburide or glibenclamide.
