Pioglitazone HCl
Indications/Uses
As monotherapy or in combination w/ a sulfonylurea, metformin or insulin in the treatment of type 2 DM not controlled by diet & exercise.
Dosage/Direction for Use
Monotherapy Patients w/o CHF Start at 15 or 30 mg once daily. Patients w/ NYHA class I or II heart failure Start at 15 mg once daily. Titrate in increments of 15 mg up to max of 45 mg once daily. Combination w/ metformin or sulfonylurea Initially 15 or 30 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Type 1 diabetes, diabetic ketoacidosis, NYHA class III or IV heart failure, serious hepatic impairment (ALT >2.5x ULN) or active liver disease, active bladder cancer. Pregnancy & lactation.
Special Precautions
Symptomatic heart failure; excessive, rapid wt gain, dyspnea, edema; history of bladder cancer, renal impairment, abnormal liver tests. Carefully monitor patients for signs & symptoms of CHF & fluid retention after initiation or dose increase. Perform liver function test prior to therapy & eye exam regularly. Consider risk of fracture especially in women & increased possibility of pregnancy (unless using contraceptive methods). Patients <18 yr.
Adverse Reactions
CV, hepatic effects; upper resp tract infection, dyspnea, sinusitis, bronchitis, pulmonary edema & pharyngitis; headache, hypoesthesia, vertigo, dizziness & insomnia; UTI, hematuria, glycosuria & proteinuria; flatulence, increased appetite & diarrhea; aggravated DM, erectile dysfunction; arthralgia, myalgia, back pain & cramps; anemia, decreased Hb & hematocrit; macular edema w/ decreased visual acuity. Tooth disorder, accidental injury, fatigue, sweating, pain in extremity, wt gain, increased lactic dehydrogenase levels.
Drug Interactions
OCs, insulin & insulin secretagogues, midazolam; strong CYP2C8 inhibitors: gemfibrozil; CYP2C8 inducers: rifampicin; drugs metabolized via CYP450: atorvastatin, nifedipine, ketoconazole.
