Azithromycin dihydrate
Indications/Uses
Infections caused by susceptible organisms. Resp tract infections (including otitis media), skin & soft-tissue infections, & uncomplicated genital infections. Prophylaxis & as a component of regimens in the treatment of Mycobacterium avium complex (MAC). Prophylaxis of endocarditis in at-risk patients unable to take penicillin. Management of trachoma & typhoid.
Dosage/Direction for Use
Adult 500 mg as single dose daily for 3 days. Initial dose: 500 mg followed by 250 mg daily for 4 days. Uncomplicated genital infections due to Chlamydia trachomatis 1 g as a single dose. Uncomplicated gonorrhea 2 g as a single dose. Prophylaxis of disseminated MAC infections 1.2 g once wkly.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Contraindications
Hypersensitivity to azithromycin, erythromycin or any macrolide antibiotics.
Special Precautions
Rare serious allergic reactions, including angioedema & anaphylaxis. Patients w/ significant hepatic disease. Do not co-administer w/ ergot derivatives. Pregnancy & lactation.
Adverse Reactions
GI disturbances. Headache, somnolence, & taste disturbances. Agranulocytosis, aggravation of muscular weakness in myasthenia gravis patients, & pancreatitis. Prolongation of QT interval & other arrhythmias, sometimes fatal, including torsades de pointes.
Drug Interactions
Reduced rate of absorption w/ antacids containing Al or Mg salts. Theoretical possibilities of ergotism w/ ergot derivatives. Impaired metabolism of digoxin (in the gut) in some patients. Increased rifabutin toxicity w/ rifabutin.
