Contents
Sultamicillin (ampicillin & sulbactam). Per 500 mg/250 mg vial Ampicillin Na 500 mg, sulbactam Na 250 mg. Per 1 g/500 mg vial Ampicillin Na 1 g, sulbactam Na 500 mg
Indications/Uses
Infections caused by susceptible microorganisms including upper & lower resp tract eg, sinusitis, otitis media, & epiglottitis, bacterial pneumonias, UTI & pyelonephritis. Intra-abdominal eg, peritonitis, cholecystitis, endometritis & pelvic cellulitis, bacterial septicemia, skin, soft tissue, bone & joint, gonococcal infections & incidence of post-op wound infections.
Dosage/Direction for Use
Adult Usual dose: 1.5-12 g/day in divided doses 6-8 hrly up to a max daily dose of 12 g. Prophylaxis for surgical infections 1.5-3 g at induction of anesth. Dose may be repeated 6-8 hrly, stopped usually 24 hr after the majority of surgical procedures. Uncomplicated gonorrhea 1.5 g as a single dose. Childn, infant & neonate 150 mg/kg/day 6-8 hrly. Neonate during the 1st wk of life (preterm) 75 mg/kg/day in divided doses 12 hrly.
Contraindications
Special Precautions
History of hypersensitivity to cephalosporins & other allergens. Discontinue use if allergic reactions & superinfections occur. Overgrowth of nonsusceptible organisms may occur. Periodically check for renal, hepatic & hematopoietic system dysfunction during extended therapy. Not to be used in infectious mononucleosis. Pregnancy & lactation.
Adverse Reactions
Inj site pain or reaction; phlebitis; anaphylactoid reaction & anaphylactic shock; nausea, vomiting, diarrhea & pseudomembranous colitis; anemia, hemolytic anemia, thrombocytopenia & leukopenia; transient increase in ALT & AST transaminases, bilirubinemia, abnormal hepatic function, jaundice; rash, itching & other skin reactions.
Drug Interactions
Increased incidence of rashes w/ allopurinol. May result in substantial mutual inactivation w/ aminoglycosides. May produce alterations in platelet aggregation & coagulation tests w/ anticoagulants. May interfere w/ bactericidal effects of penicillin w/ bacteriostatic drugs (chloramphenicol, erythromycin, sulfonamides & tetracyclines). Reduced effectiveness of estrogen-containing OCs. Decreased clearance of methotrexate. Decreased renal tubular secretion w/ probenecid resulting to increased & prolonged serum conc, prolonged elimination t½ & increased risk of toxicity. False +ve glycosuria in urinalysis using Benedict's/Fehling's reagent & Clinitest. Transient decrease in plasma conc of total conjugated estriol, estriol-glucuronide, conjugated estriol & estrone in pregnant women.
