Timolol maleate
Indications/Uses
Treatment of intraocular hypertonia eg, glaucoma; intraocular HTN.
Dosage/Direction for Use
Look upwards & pull the lower lid of the eye down slightly & instill 1 drop on affected eye. Then, press finger on the corner of the eye next to the nose for 2 min. Apply bid (morning & evening). Childn Instill 1 drop on affected eye once daily. May adjust frequency to bid at 12-hr interval if does not provide sufficient control of the pressure.
Contraindications
Allergy. Cardiac insufficiency not controlled by medication; cardiogenic shock; Prinzmetal's angina; sinus dysfunction of the heart (including SA block); bradycardia (pulse <45-50 beats/min); cardiac conduction disorders (2nd & 3rd degree AV block); Raynaud's disease & peripheral circulatory disturbances; untreated phaeochromocytoma; hypotension; treatment w/ floctafenine, sultopride; asthma & chronic pulmonary & bronchial disease w/ obstruction. In combination w/ amiodarone, certain Ca antagonists (bepridil, verapamil, diltiazem) & β-blockers for heart failure.
Special Precautions
Avoid touching eye or eyelids w/ the tip. Do not abruptly stop treatment. Do not inj. Do not swallow. Periodic ophth exam to check for drug resistance. CHD, hypotension; heart rhythm disorders eg, slow pulse; resp problems, asthma or chronic pulmonary or bronchial disease w/ obstruction; poor blood circulation (eg, Raynaud's syndrome or disease); diabetes; overactive thyroid; phaeochromocytoma; psoriasis. Patient has to undergo surgery. May induce +ve reaction in anti-doping tests. If prescribed w/ another eye drops, instill 15 min before using Timabak eye drops. May affect the ability to drive or operate machines. Do not use in pregnancy & lactation. Childn, newborns, infants.
Adverse Reactions
Generalised allergic reactions, hives, localised or generalised skin eruptions, itching, sudden onset & serious allergic reaction; hypoglycaemia; insomnia, depression, nightmares, loss of memory; loss of consciousness, strokes, decreased blood flow to the brain, increase in signs & symptoms of myasthenia gravis, dizziness, unusual sensations & headaches; ocular irritation, inflammation of the eyelid, conjunctival redness, conjunctivitis, inflammation of the cornea, blurred vision & detachment of the sub-retinal layer containing blood vessels following filtration surgery, decreased corneal sensitivity, dry eyes, corneal erosion, upper eyelid sagging, double vision, visual disorders including changes in the refraction index; slow pulse, chest pain, palpitations, oedema, changes in heart rhythm & rate, CHF, heart rhythm disorder, heart attack, heart failure; hypotension, Raynaud's phenomenon, cold hands & feet, claudication; constriction of the pulmonary airways, breathing difficulties, coughing; taste disorders, nausea, indigestion, diarrhoea, dry mouth, abdominal pain, vomiting; hair loss, psoriasis-like outbreaks or worsening of psoriasis, skin eruptions; muscular pain, SLE.
Drug Interactions
May affect or be affected by other medicines, particularly other eye drops for the treatment of glaucoma. Medicines for hypotension, heart problems or diabetes, quinidine or antidepressants eg, fluoxetine & paroxetine.
