Indications/Uses
Treatment & prophylaxis of viral infections due to HSV types 1 & 2, & varicella-zoster virus (herpes zoster & chickenpox).
Dosage/Direction for Use
Primary HSV, genital herpes 200 mg, 5 times daily (usually every 4 hr) for 5-10 days. Recurrence 800 mg daily in 2-4 divided doses, can be reduced to 400-600 mg daily or increase to 1 g daily. Therapy should be interrupted every 6-12 mth. Chickenpox 800 mg, 4-5 times daily for 5-7 days. Herpes zoster 800 mg, 5 times daily for 7-10 days. Severely immunocompromised patient or w/ impaired absorption 400 mg, 5 times daily for 5 days. Prophylaxis 200-400 mg qid. Recurrence 200 mg, 5 times daily for 5 days, preferably initiated during prodromal period.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Special Precautions
Adverse Reactions
Increased serum bilirubin & liver enzymes, hematological changes, skin rashes including erythema multiforme, Stevens-Johnson syndrome & toxic epidermal necrolysis, fever, headache, dizziness, GI effects eg, nausea, vomiting & diarrhea, anaphlaxis, thrombotic thrombocytopenic purpura, hemolytic uremic syndrome, accelerated diffuse hair loss.
Drug Interactions
Renal clearance is blocked by probenecid. Renal impairment is increased by nephrotoxic drugs.
