Per unit-dose vial Ipratropium Br 0.5 mg, salbutamol 2.5 mg
Indications/Uses
Management of reversible bronchospasm associated w/ obstructive airway diseases in patients who require more than a single bronchodilator.
Dosage/Direction for Use
Adult (including the elderly) Acute attack 1 unit-dose vial. Severe case: May require a 2nd unit-dose vial. Maintenance: 1 unit-dose vial tds or qds.
Contraindications
Hypersensitivity to atropine or its derivatives. Hypertrophic obstructive cardiomyopathy or tachyarrhythmia.
Special Precautions
Immediate hypersensitivity reactions. Immediately discontinue if paradoxical bronchospasm occurs. Patients predisposed to glaucoma; insufficiently controlled DM, recent MI, severe organic heart or vascular disorders, hyperthyroidism, phaeochromocytoma, risk of narrow-angle glaucoma, prostatic hypertrophy or bladder neck obstruction; w/ underlying severe heart disease; hypoxia; cystic fibrosis. Acute, rapidly worsening dyspnoea. May lead to +ve results in nonclinical substance abuse tests (eg, doping). Possible dizziness, accommodation disorder, mydriasis & blurred vision; observe caution when driving a car or operating machinery. Patients w/ hepatic or renal insufficiency. Pregnancy (especially during 1st trimester) & lactation. Not indicated for paed patients.
Adverse Reactions
Headache, throat irritation, cough, dry mouth, GI motility disorders (including constipation, diarrhea & vomiting), nausea & dizziness.
Drug Interactions
Increased side effects w/ xanthine derivatives, other β-adrenergics & anticholinergics. Increased β2-agonist induced hypokalaemia w/ xanthine derivatives, glucocorticosteroids & diuretics. Increased susceptibility to arrhythmia w/ digoxin; monitor serum K levels. Potentially serious reduction in bronchodilator effects w/ β-blockers. β-adrenergic agonists action may be enhanced w/ MAOIs or TCAs. Susceptibility to CV effects of β-agonists may be increased w/ halogenated hydrocarbon anaesth eg, halothane, trichloroethylene & enflurane.
