Diacerein
Indications/Uses
Degenerative joint diseases (OA & related diseases). Symptomatic relief or improvement of OA. Long-term treatment can slow down disease progression & preserve articular cartilage. Alternative medication in patients who do not respond to other OA treatments &/or who present CI to joint replacement/surgery, NSAIDs & those w/ CV problems in whom NSAIDs are contraindication.
Dosage/Direction for Use
1 cap once daily or bid for at least 3 mth. Initially, 1 cap daily for 2-4 wk (in concomitant use w/ NSAID or analgesic for pain relief), then may be increased to 1 cap bid after being accustomed to the treatment.
Administration
Should be taken with food: Take w/ main meals.
Contraindications
Hypersensitivity to diacerin, or history of hypersensitivity to anthraquinone derivatives. Previous episodes of enterocolic disturbances eg, irritable colon. Inflammatory intestinal disease (eg, ulcerative colitis, Crohn's disease), intestinal or pseudo-obstruction, painful abdominal syndromes of undetermined cause & severe liver disease.
Special Precautions
Discontinue if unusual frequent liquid or watery stools; symptoms suggestive of liver damage occurs. Dehydration & hypokalemia may occur during first 2-4 wk of treatment in patients receiving diuretics. Particular caution in case of hypokalemia in patients treated w/ cardiac glycosides (digitoxin, digoxin). Not to be taken concomitantly w/ laxatives especially during 1st 2-4 wk of treatment. Concomitant use w/ other medicinal products associated w/ hepatic injury. Limit alcohol intake during treatment. Severe renal insufficiency (CrCl <30 mL/min). History of liver disease. Not to be administered in pregnancy & lactation. Childn <18 yr. Elderly ≥65 yr.
Adverse Reactions
Diarrhea, abdominal pain, frequent bowel movements & flatulence. More intense yellow coloring of the urine. Pruritus, rash & eczema.
Drug Interactions
Concomitantly w/ drugs that modify intestinal transit &/or quality of the intestinal content (eg, excess fibers or phytates). Concomitant use of Mg, Al or Ca-based antacids. Increased in enterocolic events w/ antibiotics (eg, amoxicillin/clavulanic acid) & chemotherapy. Increased risk of arrhythmia w/ diuretics (high-ceiling loop & thiazides) &/or cardiac glycosides (digitoxin, digoxin).
