Contents
Per 1,000 mL [after mixing 2 soln from upper (amino acid soln w/ electrolytes) & lower (glucose soln w/ electrolytes & vit B1) chamber] Electrolytes: Na 35 mEq, K 20 mEq, Mg 5 mEq, Ca 5 mEq, Cl 35 mEq, sulfate 5 mEq, acetate 16 mEq, L-lactate 20 mEq, citrate 6 mEq, P 10 mmol, Zn 5 μmol. Carbohydrate: Glucose 75 g, glucose conc 7.5%. Amino acids: Total free amino acids 30 g, total nitrogen 4.7 g, essential/non-essential amino acids: 1.44, branched-chain amino acids 30% w/w. Vit: Thiamine Cl HCl 1.92 mg (thiamine equiv 1.5 mg). Total calories: 420 kCal, non-protein calories: 300 kCal, non-protein calories/nitrogen: 64
Indications/Uses
Provision of amino acids, electrolytes, vit B1 & water via peripheral vein to patients w/ mild hypoproteinemia or malnutrition due to inadequate oral intake, before & after surgery.
Dosage/Direction for Use
Adult 500 mL/dose, infused via peripheral vein over 120 min. Max dose: 2,500 mL daily.
Contraindications
Hypersensitivity to thiamine Cl HCl. Patients w/ or at risk of developing hepatic coma; serious renal dysfunction or azotemia; CHF; severe acidosis; abnormal electrolyte metabolism (hyperkalemia, hyperphosphatemia, hypermagnesemia, hypercalcemia); reduced urine output due to obstructive uropathy; abnormal amino acid metabolism.
Special Precautions
Hypersensitivity. Discontinue administration or use an alternate site if vascular pain occurs. Immediately discontinue if there are clinical signs of extravasation at the inj site. Use only for short-term nutritional therapy. Assess overall nutritional requirement & oral intake. Limit use to 3-5 days when used alone in post-op patients & institute oral/enteral nutrition or other regimens if feasible. Administer additional vit B1 or other vit depending on the condition. Patients w/ hepatic, renal & CV dysfunction; acidosis; DM. Pregnancy. Avoid use during lactation. Childn. Reduce administration rate in seriously ill patients; elderly.
Adverse Reactions
Vascular pain, phlebitis. Nausea, vomiting; chest discomfort; elevated AST, ALT, alkaline phosphatase & total bilirubin levels; shock.
