Indications/Uses
Central & peripheral neuropathic pain in adults, including diabetic peripheral neuropathy & post-herpetic neuralgia. Generalized anxiety disorder (GAD) in adults. Management of fibromyalgia. Adjunctive therapy in adults w/ partial seizures w/ or w/o secondary generalization.
Dosage/Direction for Use
Neuropathic pain Initially 150 mg daily. May be increased to 300 mg daily after 3-7 days interval. Max daily dose: 600 mg after an additional 7-day interval. Epilepsy Initially 150 mg daily. May be increased to 300 mg daily after 1 wk. Max daily dose: 600 mg may be achieved after an additional wk. GAD Initially 150 mg daily. May be increased to 300 mg daily after 1 wk, then further increased to 450 mg daily following an additional wk. Max daily dose: 600 mg may be achieved after an additional wk. Fibromyalgia Initially 75 mg bid (150 mg daily) & may be increased to 150 mg bid (300 mg daily) w/in 1 wk based on efficacy & tolerability. Patients who do not experience sufficient benefit w/ 300 mg daily May be further increased to 225 mg bid (450 mg daily). Max daily dose: Patients based on individual response & tolerability If needed, 600 mg after an additional wk. Renal impairment CrCl ≥60 mL/min Initial dose: 150 mg daily, max dose: 600 mg daily; CrCl ≥30-<60 mL/min Initial dose: 75 mg daily, max dose: 300 mg daily. All doses are given in 2 or 3 divided doses; CrCl ≥15-<30 mL/min Initial dose: 25-50 mg daily, max dose: 150 mg daily. All doses are given in single daily dose or 2 divided doses; CrCl <15 mL/min Initial dose: 25 mg daily, max dose: 75 mg daily. All doses are given in single daily dose. Supplementary dosage following hemodialysis Initial dose: 25 mg, max dose: 100 mg. All doses are given in single additional dose. Supplementary dose should be given immediately following every 4-hr hemodialysis treatment.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Discontinue immediately if symptoms of angioedema eg, facial, perioral, or upper airway swelling occur. Observe patients for signs & symptoms of drug misuse, abuse or dependence (eg, tolerance development, dose escalation, drug-seeking behavior). Patients w/ current substance abuse or history of substance abuse, who are at higher risk for drug abuse; liver & kidney disease. CHF patients. May cause hematologic disorder. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Gradually discontinue treatment over a minimum of 1 wk. Avoid alcoholic beverages during treatment. Concomitant use w/ opioids. May affect ability to drive & use machines. Women of childbearing potential must use effective contraception during treatment. May cause teratogenesis. Not to be used during pregnancy. Discontinue breastfeeding or discontinue from therapy. Not recommended in childn. Elderly >65 yr due to decreased renal function.
Adverse Reactions
Dizziness, somnolence; headache. Nasopharyngitis; increased appetite; euphoric mood, confusion, irritability, depression, disorientation, insomnia, decreased libido; ataxia, abnormal coordination, tremor, dysarthria, amnesia, memory impairment, disturbance in attention, paraesthesia, hypoesthesia, sedation, balance disorder, lethargy; blurred vision, diplopia; vertigo; vomiting, constipation, flatulence, abdominal distension, dry mouth; muscle cramp, arthralgia, limb & back pain, cervical spasm; peripheral oedema, oedema, abnormal gait, fall, feeling drunk & abnormal, fatigue; increased wt; nausea, diarrhea. SJS.
Drug Interactions
May potentiate effects of ethanol & lorazepam. Additive in impairment of cognitive & gross motor function caused by oxycodone. Concomitant use w/ other CNS depressants. Reduced lower GI function w/ medications that have potential to produce constipation eg, opioid analgesics.
