Cilostazol
Indications/Uses
Symptomatic management of peripheral vascular disease primarily intermittent claudication. Adjunct w/ aspirin & clopidogrel for prevention of stent thrombosis & restenosis after coronary stent placement.
Dosage/Direction for Use
Adult 100 mg bid. Concomitant use w/ CYP2C19 & CYP3A4 inhibitors Reduce dose to 50 mg bid.
Administration
Should be taken on an empty stomach: Take at least 30 min before or 2 hr after meals.
Contraindications
Hypersensitivity. Heart failure. Hemostatic disorders or active bleeding. Pregnancy, lactation.
Special Precautions
Moderate to severe hepatic & severe renal impairment. Patients receiving other platelet aggregation inhibitors or w/ thrombocytopenia. Discontinue therapy if thrombocytopenia or leukopenia occur. Concomitant use w/ clopidogrel. Withhold for at least 4-6 half-lives prior to elective surgical procedures. Receiving CYP3A4 inhibitors (eg, ketoconazole or erythromycin) or CYP2C19 inhibitors (eg, omeprazole). Severe underlying heart disease. Ped patients.
Adverse Reactions
Headache; abnormal stools, diarrhea; rhinitis; infection.
Drug Interactions
Serum conc may be increased by antifungal agents, macrolide antibiotics & omeprazole. Increased conc w/ CYP3A4 or CYP2C19 inhibitors. May potentiate platelet aggregation inhibition of aspirin. Increased bleeding w/ anticoagulants, dasatinib, drotrecogin α, NSAIDs or treprostinil. Decreased levels/effects by CYP3A4 inducers & St. John's wort. Potential inhibition of CYP3A4 w/ grapefruit juice. Avoid alfalfa, anise, bilberry, bladderwrack, bromelain, cat's claw, chamomile, coleus, cordyceps, dong quai, evening primrose oil, fenugreek, feverfew, garlic, ginger, Ginkgo biloba, ginseng (American, Panax, Siberian), grape seed, green tea, guggul, horse chest nut seed, horse radish, licorice, prickly ash, red clover, reishi, sweet clover, turmeric, white willow, S-adenosylmethionine.
