Entacapone
Indications/Uses
Adjunct to levodopa/benserazide or levodopa/carbidopa treatment in patients w/ Parkinson's disease & end-of-dose motor fluctuations, who cannot be stabilized on those combinations.
Dosage/Direction for Use
1 tab w/ each levodopa/dopa decarboxylase inhibitor dose. Max: 2,000 mg/day.
Administration
May be taken with or without food.
Contraindications
Liver impairment. Pheochromocytoma. Concomitant use w/ nonselective MAO-A & MAO-B inhibitors. Concomitant use w/ a selective MAO-A plus a selective MAO-B inhibitor. History of NMS &/or non-traumatic rhabdomyolysis. Pregnancy & lactation.
Special Precautions
Levodopa-induced orthostatic hypotension may be aggravated. Avoid abrupt w/drawal; discontinue gradually when necessary.
Adverse Reactions
Dyskinesia; GI symptoms; urine discoloration; fatigue, insomnia, paroneiria, confusion, hallucinations; dizziness, postural hypotension, vertigo, headache; leg cramps, hyperkinesia, tremor, increased sweating; slight decrease in Hb, erythrocyte count & hematocrit. Increases in liver enzymes (rare). NMS, especially following abrupt reduction/discontinuation of entacapone, & rhabdomyolysis (isolated cases).
Drug Interactions
Entacapone & Fe prep should be taken at least 2-3 hr apart. Dosage of levodopa & other antiparkinsonian drugs may need to be adjusted when entacapone treatment is initiated. Effects of medicinal products metabolized by catechol-O-methyl transferase may be potentiated.
