Indications/Uses
Established postmenopausal osteoporosis; renal osteodystrophy in patients w/ chronic renal failure, particularly those undergoing hemodialysis; post-surgical hypoparathyroidism; idiopathic hypoparathyroidism; pseudohypoparathyroidism; vit D-dependent rickets; hypophosphatemic vit D-resistant rickets.
Dosage/Direction for Use
Postmenopausal osteoporosis 0.25 mcg bid. Renal osteodystrophy (dialysis patients) Initially 0.25 mcg daily. Patient w/ normal or slightly reduced serum Ca levels 0.25 mcg every other day. May be increased by 0.25 mcg daily for 2-4 wk intervals if no satisfactory response is observed w/in 2-4 wk. Hypoparathyroidism & rickets Initially 0.25 mcg daily in the morning. May be increased at 2-4 wk intervals if satisfactory response in biochemical parameters & clinical manifestations of disease are not observed.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. All diseases associated w/ hypercalcemia.
Special Precautions
Chronic hypercalcemia w/ increased serum creatinine. Immobilized patients eg, those who have undergone surgery. Avoid dehydration & maintain adequate fluid intake. Check serum Ca levels every mth. Determine serum Ca & phosphate levels daily during periods of hypercalcemia; serum Ca & creatinine levels at 4 wk, 3 & 6 mth, & at 6 mthly interval thereafter in postmenopausal osteoporosis; serum Ca levels at least twice wkly in hypoparathyroidism & rickets. Possible stimulation of intestinal absorption of phosphate. Avoid other vit D prep. Renal failure (ectopic calcification). Pregnancy. Not to breastfeed during therapy.
Adverse Reactions
Hypercalcemia syndrome or Ca intoxication. Anorexia, headache, vomiting, constipation. Dystrophy, sensory disturbances, fever w/ thirst, polyuria, apathy, arrested growth, UTI.
Drug Interactions
Possible additive effects & hypercalcemia w/ vit D & its derivatives. Avoid concomitant use w/ Ca supplements & Ca-containing prep. Increased risk of hypercalcemia w/ thiazide diuretics. Precipitated cardiac arrhythmias w/ digitalis. Concomitant use w/ corticosteroids.; Mg-containing drug eg, antacids; phosphate binding agents. Reduced serum conc w/ enzyme inducers eg, phenytoin or phenobarb. Impaired intestinal absorption w/ cholestyramine.
